Improvement of Type 2 Diabetes (blood sugar control)
This medication is used to improve blood sugar control in patients with Type 2 diabetes. It is more effective when taken alongside dietary and exercise therapy. It can be used alone but may also be combined with sulfonylureas, metformin, or insulin if adequate effects are not obtained from other oral hypoglycemic agents. Additionally, for patients with Type 2 diabetes, nutritional counseling, weight loss, and regular exercise are vital for enhancing treatment efficacy. These approaches aid in maintaining the efficacy of drug therapy as well as in initial treatment. Therefore, this drug is effectively utilized to improve blood sugar control.
This drug is taken orally once a day, regardless of meals. It can be started as a single therapy at 15 mg once daily, and if not adequately effective, it may be increased to 30 mg once daily. In combination therapy, it is administered at 15 mg once daily with sulfonylureas, metformin, or insulin, with the maximum recommended dosage not exceeding 30 mg once daily.
There is no experience in use for children under 18 years old, so administration is not recommended.
This drug can cause or worsen cardiac dysfunction known as congestive heart failure in some patients. After initiating treatment or increasing the dose, it is crucial to monitor for symptoms of heart failure, such as sudden significant weight gain, shortness of breath, or swelling. If any symptoms of heart failure occur, immediate medical attention should be sought, and the drug may need to be discontinued or the dose reduced if necessary. Patients with heart failure or a history of heart failure should not use this drug. When used in conjunction with insulin, this drug may cause fluid retention and worsen heart failure, so patients with heart disease should exercise particular caution. If symptoms such as hematuria, urinary problems, or urgency arise during treatment, they may indicate bladder cancer, and immediate notification to a physician is necessary. This drug should not be used in patients with poor liver function, severe kidney disease, diabetic ketoacidosis, Type 1 diabetes, active bladder cancer, and others. There is a risk of hypoglycemia when used with other antidiabetic agents, so it is essential to be aware of hypoglycemia symptoms and management, and adjust the doses of other medications if necessary. This drug can cause weight gain and edema, so periodic monitoring of weight changes and edema symptoms is necessary. For premenopausal women, this drug may stimulate ovulation, increasing the risk of pregnancy, so using appropriate contraceptive methods is advised. Regular blood sugar and liver function tests during treatment are vital, and any symptoms of liver damage (fatigue, loss of appetite, jaundice, etc.) should be reported to a doctor immediately. Elderly patients, especially those over 75 years of age, should exercise increased caution when using this drug, as they may have a higher likelihood of adverse reactions. This drug may affect the fetus during pregnancy, so women who are pregnant or may become pregnant should avoid using it, and breastfeeding individuals should consult their doctor. In the event of overdose, specific antidotes are not available, so appropriate treatment should be administered based on symptoms. The safety and efficacy of this drug in children under 18 years of age have not been established, so administration is not recommended. This drug's blood concentration may vary when taken with other medications, so consult a physician when starting or stopping any other drugs.
