This medication is used for the improvement of atherosclerotic symptoms in adult patients with ischemic stroke, myocardial infarction, or peripheral artery disease. It is also effective in improving symptoms of cardiovascular abnormalities, including the risk of death, myocardial infarction, stroke, or ischemic symptoms in patients with unstable angina or non-Q-wave myocardial infarction who have received or will receive medical treatment, coronary interventions, or coronary artery bypass surgery. Furthermore, in patients with atrial fibrillation who have one or more vascular risk factors and where vitamin K antagonists are not suitable and the risk of bleeding is low, it is used to reduce the risks of stroke and thromboembolism. Hence, it is a drug that helps improve symptoms and reduce risks in various cardiovascular and cerebrovascular-related diseases.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral artery disease, clopidogrel is administered orally at a dose of 75 mg once daily. For patients with acute coronary syndrome, a loading dose of 300 mg is given once daily on the first day, followed by a maintenance dose of 75 mg orally once daily. Aspirin should be co-administered at a dose of 75 to 325 mg once daily during this time. For patients with atrial fibrillation, 75 mg of this drug should be taken orally once daily.

Precautions

This medication prevents platelets from aggregating, thereby preventing blood clotting. It acts after being converted into its active form in the liver, and individuals genetically reduced in the hepatic enzyme CYP2C19 may experience reduced efficacy. Thus, individuals genetically having impaired function of this enzyme should consider changing their treatment methods or substituting with another drug. Additionally, when taken with medications that activate the CYP2C19 enzyme, the concentration of this drug's active metabolites may increase, raising the bleeding risk, hence it should be avoided in combination. This drug should not be used in patients with severe liver impairment, those with bleeding, patients who are allergic to this drug or its components, lactating women, or those with lactose intolerance. In cases of high bleeding risk or if bleeding symptoms are suspected, blood tests should be performed promptly, and caution should be exercised in patients with other blood coagulation abnormalities or those scheduled for surgery. Particularly, when using this drug alongside aspirin, the risk of bleeding is increased, so co-administration should be carefully considered. During the course of taking this drug, the time it takes for bleeding to stop can be longer than usual, so if bleeding does not stop or there are unusual bleeding symptoms, it should be reported to medical personnel immediately. Rarely, thrombocytopenia or severe blood abnormal reactions may occur, so testing should be done quickly for any signs of infection or bleeding symptoms. This drug may cause side effects such as gastrointestinal disturbances, diarrhea, headache, rash, and rarely severe bleeding or allergic reactions. Pregnant women should avoid use unless in emergencies, and lactating women should consult their physician as the drug may be excreted in breast milk. Elderly patients or those with diminished kidney and liver function are at increased risk of bleeding; therefore, dosage adjustment and monitoring of their condition are necessary. Overdosage can significantly increase the risk of bleeding, and as there is no specific antidote, prompt medical treatment and platelet transfusion may be required. This drug may interact with other medications that pose a bleeding risk, anticoagulants, non-steroidal anti-inflammatory drugs, certain antibiotics, and antidepressants, so all medications being taken must be disclosed. Finally, store this medication out of reach of children and ensure it is not transferred to other containers arbitrarily.
