This drug is used to improve symptoms caused by atherosclerosis in adult patients with ischemic stroke, myocardial infarction, and peripheral arterial disease. It also helps reduce the risk of cardiovascular complications in patients with acute coronary syndrome, specifically those with unstable angina or non-ST elevation myocardial infarction, who are receiving medical treatment, coronary intervention, or coronary artery bypass surgery. Additionally, it is used to decrease the risk of stroke and thromboembolism in patients with atrial fibrillation who have vascular risk factors and for whom vitamin K antagonists are not appropriate. Overall, this medication effectively improves the risks and symptoms of cardiovascular and cerebrovascular diseases.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer clopidogrel at a dosage of 75 mg orally once daily. For patients with acute coronary syndrome (unstable angina or non-ST elevation myocardial infarction), initiate therapy with a loading dose of 300 mg on the first day, followed by a maintenance dosage of 75 mg orally once daily. During this time, aspirin 75 to 325 mg should be co-administered once daily.

Precautions

This drug helps prevent platelets from aggregating, allowing blood to flow more freely. Caution is warranted for individuals with genetically reduced function of a specific enzyme, CYP2C19, as this may decrease the drug's effectiveness. Moreover, co-administration of this drug with medications that activate the CYP2C19 enzyme may increase the risk of bleeding, thus concurrent use should be avoided. Patients with a known allergy to this drug or its components, patients with bleeding disorders, severely damaged liver function, nursing mothers, and individuals with certain genetic conditions should not take this medication. Patients at high risk for bleeding or those scheduled for surgery should consult with their physician about medication management, particularly when taking other antiplatelet agents or anticoagulant medications, as additional caution is required. This medication may cause bleeding, thus it is important to immediately inform healthcare personnel of any signs or symptoms of bleeding that occur. It is necessary to regularly monitor for any bleeding or blood abnormalities through blood tests or other required examinations. This drug may result in various side effects such as gastrointestinal disturbances, headache, dizziness, rash, fatigue, and if symptoms are severe or persistent, consultation with a doctor is necessary. Particularly, bleeding-related side effects are the most common; therefore, it is crucial to notify healthcare personnel immediately if symptoms such as nosebleeds, gum bleeding, bruising, hematuria, or coughing up blood occur. If pregnant or breastfeeding, it is important to discuss any potential effects of this drug on the fetus or infant with a physician, and whether to continue breastfeeding while taking the medication should be carefully considered. Children and the elderly may have different safety and effectiveness profiles for this drug; special caution is recommended, especially for the elderly, as they may have a higher risk of bleeding and should take the medication as directed by their physician. Overdosing on this drug can increase bleeding risk, so it is essential to adhere strictly to the prescribed dosage. In case of overdose, it is imperative to go to the hospital immediately. This medication should be kept out of the reach of children and it is advisable to avoid transferring it to other containers. Finally, when starting new medications or planning surgeries while taking this drug, it is essential to inform healthcare personnel about its use.
