Essential hypertension is a condition of increased blood pressure without a specific cause, requiring appropriate treatment for heart and vascular health. This medication helps to stably lower blood pressure, reducing the burden on blood vessels and aiding in the prevention of complications such as heart disease and stroke. Regular use maintains blood pressure control and improves overall cardiovascular health. Additionally, lifestyle modifications are also recommended alongside drug therapy. This medication is used for the treatment of essential hypertension.

How to take

Adult dose

The recommended initial dose is 10mg once daily, regardless of meals. If blood pressure is not adequately controlled, it may be increased to 20mg once daily. The maximum allowable dose is 40mg per day. There is no need for dosage adjustment in elderly patients (aged 65 and older). For mild and moderate renal impaired patients (creatinine clearance 20-60mL/min), the maximum dose is 20mg once daily.

Precautions

This medication can cause serious harm or death to the fetus and newborn if taken during the second and third trimester of pregnancy, so it must be discontinued immediately upon confirmation of pregnancy. Patients who are allergic to any component of this medication, are pregnant or breastfeeding, are undergoing kidney dialysis, or have severe liver disease or biliary obstruction should not take this medication. Patients with specific genetic issues such as lactose intolerance should also avoid this medication. Patients with decreased blood volume or sodium may experience hypotension, so it is important to check their condition carefully before use and monitor hypotension symptoms during use. Patients with severe heart failure or impaired renal function may have worsening kidney function when taking this medication, thus regular blood tests and monitoring are necessary. Caution is also needed for patients with renal vessel problems, those at risk for hyperkalemia, and those with certain heart conditions. Dizziness may occur during treatment, so driving or operating hazardous machinery should be avoided. While this medication is generally safe for the treatment of hypertension in most patients, some may experience side effects such as dizziness, headache, abdominal pain, and muscle pain. Rarely, serious side effects such as angioedema, liver function abnormalities, hyperkalemia, and acute renal failure can occur, so any unusual symptoms should be reported to a doctor immediately. During treatment, care should be taken when using other antihypertensive agents, potassium-sparing diuretics, or potassium supplements, as blood potassium levels may increase. Co-administration with lithium formulations may result in elevated lithium levels and toxicity, so they should not be taken together. Co-administration with non-steroidal anti-inflammatory drugs may worsen kidney function, so kidney function should be monitored regularly during treatment. Women who are pregnant or breastfeeding should avoid this medication, especially during pregnancy due to the serious risk to the fetus, requiring discontinuation of the medication. Safety for use in children has not been established, thus it is not used in pediatric patients. Elderly patients may experience increased drug concentrations and thus caution is advised during use. Overdose may lead to hypotension or arrhythmias, requiring appropriate treatment. This medication has been reported to have a low risk of carcinogenicity or genotoxicity even with long-term use, but any unusual symptoms during use should be discussed with a doctor.
