Efficacy & effects

Adjunctive treatment for weight loss in patients with obesity (high body mass index and risk factors)

  • Obesity
View approved original text

This medication is used for obese patients who do not respond to appropriate dietary and exercise regimens. It is particularly applicable for individuals with a body mass index (BMI) of 30 kg/m2 or higher, or those with risk factors such as hypertension, diabetes, and hyperlipidemia with a BMI of 27 kg/m2 or higher. It is used as a short-term adjunctive treatment for calorie-restricted weight loss therapy, and this medication should be used alone without combining it with other appetite suppressants. Therefore, this medication plays an effective adjunctive role in managing obesity and helps with weight loss.

Dosage and Administration

Adult dosage

Phendimetrazine Tartrate: 35 mg orally, 2 to 3 times a day, taken 1 hour before meals. The dosage should be the minimum effective dose, adjusted individually to achieve appropriate response. For some patients, 17.5 mg once a day may be appropriate. The dose should not exceed 70 mg at any single time, and this medication is given for a short term (within 4 weeks).

Precautions

This medication must be used alone and for a short duration only, as using it in combination with other appetite suppressants can lead to serious cardiac issues. Long-term use of this medication can significantly increase the risk of serious pulmonary conditions such as pulmonary arterial hypertension. If symptoms such as shortness of breath, chest pain, fainting, or leg swelling occur, discontinue use immediately and visit a hospital. Individuals with heart diseases such as valvular heart disease should undergo cardiac testing before using this medication, and it is not recommended for patients with cardiac murmurs or valve issues. This medication should not be used by individuals with certain heart conditions, hypertension, hyperthyroidism, glaucoma, mental anxiety, history of substance abuse, children under 16 years old, or women who are pregnant or may become pregnant. Additionally, it should not be taken with other central nervous system stimulants or specific antidepressants, and individuals who are hypersensitive to this medication should avoid it. Those with mild hypertension, diabetes, or women who are breastfeeding should use this medication with special caution. This medication may cause side effects such as palpitations, anxiety, dizziness, insomnia, gastrointestinal disturbances, changes in sexual desire, frequent urination, urinary retention, and erectile dysfunction, so one should consult a doctor if symptoms are severe. Tolerance to the appetite-suppressing effect typically develops within a few weeks, so if the effect diminishes, do not increase the dose but rather discontinue use. As this medication affects the central nervous system, caution is needed while driving or operating hazardous machinery. Diabetics may need to adjust their insulin dosage while taking this medication. In case of overdose, serious symptoms such as anxiety, coma, seizures, and abnormal blood pressure may occur; if this happens, immediate emergency treatment is necessary. Keep this medication out of reach of children, as it is classified as a narcotic and has abuse potential; therefore, it should be used cautiously under a physician's guidance. If this medication has been used long-term at high doses and then is suddenly discontinued, severe fatigue and depression may occur, so it should be tapered off slowly.
