This medication is primarily used to effectively lower blood pressure in patients with essential hypertension. It also helps improve symptoms in heart failure patients (NYHA class II to IV) who are intolerant to ACE inhibitors. In stable patients with left ventricular dysfunction or impaired systolic function after a myocardial infarction, it is also used to reduce the risk of mortality. It positively impacts various cardiovascular diseases and improves patients' health.

Dosage and Administration

Adult dosage

Essential Hypertension: Taken with water during meals or between meals. It is recommended to take 80 mg orally once daily at the same time each day (e.g., in the morning). If blood pressure control is insufficient, the dose may be increased to 160 mg daily, or a diuretic may be used in conjunction. For additional blood pressure reduction, it may be increased to a maximum of 320 mg daily. Elderly patients (aged 75 and older) should start with 40 mg once daily.

Precautions

This medication can cause serious harm to the fetus if taken during pregnancy, especially in the second and third trimesters, so it must be discontinued immediately upon confirmation of pregnancy. It is also not safe for women who may become pregnant and for nursing mothers to use this medication. Patients with an allergic reaction to this medication or severe liver diseases and significant renal impairment should not use this medication. Elderly patients, those with high serum potassium levels, and those with mild to moderate liver impairment should exercise caution when using this medication. Patients with heart diseases or those undergoing surgery should be closely monitored while on this medication. Blood pressure may drop sharply during treatment, potentially leading to dizziness or fainting; symptoms should be reported to healthcare professionals immediately. Additionally, caution must be taken when used alongside potassium-sparing diuretics or potassium supplements, as this can dangerously elevate serum potassium levels. Other antihypertensives or NSAIDs may alter the effect, and consultation with a physician is necessary. The safety for children and adolescents has not been established, therefore use is not recommended. In cases of overdose, severe hypotension or decreased consciousness may occur, necessitating immediate emergency care. The medication should be kept out of reach of children, and it is advisable not to transfer it to other containers. During treatment, regular monitoring of blood pressure, renal function, and serum potassium levels is essential to check for any abnormalities. There may be dizziness when driving or operating machinery, so caution is advised.
