This medication is used to improve atherosclerotic symptoms in adults with ischemic stroke, myocardial infarction, and peripheral arterial disease. It also helps reduce the risk of cardiovascular complications in patients with acute coronary syndrome who have unstable angina or non-Q-wave myocardial infarction and have received drug therapy, coronary intervention, or coronary artery bypass surgery. Finally, it reduces the risk of stroke and thromboembolism in patients with atrial fibrillation who have vascular risk factors and for whom vitamin K antagonists are not suitable. This medication is effectively used for the treatment of cardiovascular diseases and the prevention of complications.

How to take

Adult dose

1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, 75 mg of clopidogrel is administered orally once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), it is recommended to start with a loading dose of 300 mg on the first day of administration, followed by a maintenance dose of 75 mg once daily. During this time, aspirin 75 to 325 mg should be administered alongside this medication once daily.

Precautions

This medication works by inhibiting platelet aggregation to prevent blood clotting. Individuals with genetically reduced function of a specific enzyme (CYP2C19) may experience reduced effectiveness of this medication, and in such cases, treatment strategies should be reconsidered. Coadministration of this medication with drugs that activate CYP2C19 can increase the concentration of this medication and raise the risk of bleeding, hence coadministration is not recommended. Individuals who are allergic to this medication or its components, those with bleeding disorders, patients with severe liver damage, and nursing mothers should not take this medication. Additionally, those with genetic diseases that prevent the digestion of lactose or allergies to soybean oil, beans, or peanuts should avoid this medication. Patients at high risk of bleeding or suspected of having bleeding symptoms should inform their doctors immediately, and if surgery is planned, it is advisable to discontinue this medication 5 to 7 days prior to the procedure. Coadministration with other antiplatelet agents, anticoagulants, nonsteroidal anti-inflammatory drugs (NSAIDs), selective serotonin reuptake inhibitors (SSRIs), etc., may increase the risk of bleeding, thus caution is necessary. For pregnant women, although there is not significant impact on the fetus, the risk of bleeding during childbirth may increase, so discontinuing this medication 5 to 7 days before delivery is recommended. For nursing mothers, it is uncertain whether this medication is excreted in breast milk, and they should consult their doctor regarding discontinuation of breastfeeding or this medication. Safety in children has not been established, thus it is advisable not to administer it to pediatric patients. Elderly individuals or patients with impaired kidney and liver function may have an increased risk of bleeding, requiring dosage adjustment and careful monitoring. Overdosage may lead to severe bleeding and there is no antidote to reverse the effects of this medication; platelet transfusion may be required in case of severe bleeding. This medication should be stored out of reach of children, and it is safer not to transfer it to another container. If any adverse symptoms such as bleeding, skin rash, difficulty breathing, severe abdominal pain, or persistent headache occur during use, immediate medical attention is required.
