Precautions

This drug is used as a local anesthetic, and a rare blood disorder called methemoglobinemia may occur with its use. Caution should be exercised particularly in patients with glucose-6-phosphate dehydrogenase deficiency or congenital methemoglobinemia, patients with cardiac or pulmonary impairment, infants under 6 months, and when administered in conjunction with oxidizing agents. The use of this drug may cause skin to turn blue or the blood color to appear abnormal, and in severe cases, serious symptoms such as seizures, coma, or arrhythmias may occur. If such symptoms are suspected, the medication should be immediately discontinued and appropriate treatment sought. This drug should not be used in patients with hypersensitivity to it, or in areas with wounds, mucosal lesions, or dermatitis, nor in infants under 3 months or infants taking certain medications, or premature infants. Caution is also warranted in patients with anemia or liver disease, and in frail or elderly patients. This medication contains propylene glycol, so caution is needed if allergic to this component. Local reactions such as skin pallor, redness, swelling, itching, or burning may occur, and rarely, serious allergic reactions such as urticaria, angioedema, bronchospasm, or shock may occur. In children with atopic dermatitis, prolonged use may rarely lead to skin reactions like purpura or petechiae. Avoid using around the eyes as it may cause corneal irritation; if contact occurs, rinse immediately with clean water or saline. Although systemic absorption is slow, caution should be exercised as toxic responses such as central nervous system symptoms or cardiac function depression may occur with excessive use. Toxicity may also increase when used in combination with other local anesthetics or certain arrhythmia treatments, hence it should be used under medical supervision. Careful use is needed during pregnancy or lactation, as this drug can cross the placenta and be secreted in breast milk. It should be used restrictively in children, especially infants under 3 months, and it is safer not to apply it to the genital mucosa. Lastly, this drug may accumulate in the body as a metabolite potentially carcinogenic if used in high doses or for extended periods, so strict adherence to the recommended dosage and duration of use is necessary.
