Precautions

This medication should not be administered to certain patients. First, patients with a history of allergies to this medication or its components, or those with a history of hypersensitivity reactions such as angioedema, should not use it. Patients with orthostatic hypotension may experience worsening symptoms, so it’s advisable not to use this medication. Patients with severely impaired liver function may have higher blood levels of the medication, so it should not be administered. Women who are pregnant or may become pregnant should also not use this medication. Additionally, it should be used with caution in some patients. Patients with severely reduced kidney function may have increased blood levels of the medication, requiring careful monitoring. Elderly patients may react differently to the medication, and dosing should be started slowly with observation. Patients taking antihypertensives, PDE5 inhibitors, or CYP3A4 inhibitors may have an increased risk of side effects due to drug interactions, so careful administration is needed. In particular, patients currently taking alpha-1 blockers should avoid this medication due to the risk of orthostatic hypotension, and it should only be used with extreme caution when absolutely necessary. Patients with a history of urinary-related syncope should likewise use this medication with caution. While taking this medication, various side effects may occur. Syncope or loss of consciousness due to low blood pressure may occur, so symptoms should be closely monitored. Symptoms related to the central nervous system such as dizziness, headaches, drowsiness, insomnia, and decreased libido may also occur. Cardiac symptoms reported include rapid heartbeat, low blood pressure, and arrhythmias. If allergic reactions like skin rash, itching, or hives occur, the medication should be stopped immediately. Gastrointestinal symptoms may include nausea, vomiting, abdominal pain, constipation, and diarrhea. Liver dysfunction or jaundice may appear, so liver function should be periodically monitored. Additionally, various adverse reactions such as nasal congestion, edema, incontinence, gynecomastia, and priapism have been reported. It is important to avoid overdose when using this medication, and especially to change positions slowly as blood pressure may drop when standing up. This medication is not a curative treatment but is intended for symptom relief, so if it is ineffective, alternative treatments should be considered. When participating in dangerous activities such as driving or working at heights, caution should be exercised due to the risk of accidents from dizziness and other side effects. As an alpha-1 blocker, serious side effects such as priapism can rarely occur, so if these symptoms arise, immediate consultation with a physician is necessary. Patients undergoing cataract or glaucoma surgery should inform their healthcare providers of potential intraoperative floppy iris syndrome. Patients with a sulfamethoxazole allergy may experience allergic reactions to this medication, so caution is necessary. When taking this medication with other antihypertensives or specific enzyme inhibitors, interactions such as blood pressure reduction or increased medication concentration can occur, and consultation with a physician or pharmacist is mandatory. Women who are pregnant or breastfeeding, and children under 18, should not use this medication as safety and efficacy have not been established. In the elderly, kidney function decline is common, so dosing should start at low levels and be closely monitored for their condition. When taking this medication, capsules should be swallowed whole, and they must not be crushed or chewed. Particularly, swallowing them in their packaging can damage the esophageal mucosa, so caution is advised.
