Improvement of blood sugar control in type 2 diabetes
This medication is used to assist in controlling blood sugar in patients with type 2 diabetes. It is administered as an adjunct to diet and exercise therapy and can also be used alone. If blood sugar control is not well managed with oral hypoglycemic agents alone, it can be combined with insulin. Additionally, it is administered in combination with metformin when sulfonylureas or metformin monotherapy is ineffective. This medication helps improve blood sugar control.
This medication is taken orally once daily. Start with 1 mg of glimepiride once daily. If necessary, increase the dose by 1 mg at intervals of 1 to 2 weeks. Generally, no additional effect is seen with doses over 4 mg, but some patients may increase the dosage up to 6 mg (or 8 mg). If 4 mg once daily fails, consider switching to insulin therapy.
This medication should not be used in patients with insulin-dependent diabetes or in those with diabetic ketoacidosis or in a comatose state. Furthermore, individuals who have had allergic reactions to this medication or its components should avoid taking it. There is no experience using this medication in patients with severe liver or kidney dysfunction, so switching to insulin treatment is recommended. It is also safe to avoid taking this medication during pregnancy or in women who could become pregnant and while breastfeeding. Patients with severe infections, or those undergoing surgery, should avoid using this medication as insulin treatment may be necessary. Those with gastrointestinal disorders such as diarrhea or vomiting should be cautious as absorption of food may be impaired, increasing the risk of hypoglycemia. This medication contains lactose, so individuals with lactose intolerance or specific genetic issues should not take it. During the first week of treatment, there is a heightened risk of hypoglycemia, so blood sugar should be checked frequently and observed carefully. Particularly, older adults, patients who are in poor nutritional status or eat irregularly, and those who frequently consume alcohol should be careful as they have a higher risk of hypoglycemia. Patients with impaired kidney or liver function may also respond more sensitively to this medication, necessitating dose adjustments. There may also be an increased risk of hypoglycemia in those with other endocrine disorders or who are taking multiple medications, so caution is warranted. Symptoms of hypoglycemia may include headache, severe hunger, nausea, fatigue, confusion, drowsiness, and in severe cases, loss of consciousness, so it is important to respond immediately to any abnormal symptoms. When first starting or changing doses, symptoms such as blurred vision or dyspepsia may occur temporarily. Regular testing is needed as liver function abnormalities or changes in blood cell counts may occur. If allergic reactions such as itching, rash, or difficulty breathing occur, inform a physician immediately and discontinue the medication if severe. It is important to maintain dietary and exercise regimens while taking this medication and regularly check blood sugar levels. If hypoglycemia occurs, quick ingestion of foods containing sugar is necessary to raise blood sugar rapidly, and it is advisable always to carry sugar. When taking this medication with others, drug interactions may affect blood sugar control, necessitating notification to the doctor or pharmacist of all medications being taken. Pregnant or breastfeeding women should avoid taking this medication and it is safer to switch to insulin treatment if necessary. In children, the safety and efficacy of this medication have not been established so it is best not to use it. Elderly patients should be careful about dose adjustments due to reduced drug clearance from impaired kidney function. In case of overdose, severe hypoglycemia may occur, requiring immediate medical attention.
