View approved original text

This medication is used for the treatment of patients with Type 2 diabetes who find it difficult to control blood sugar solely with diet and exercise, especially for those who are overweight. In adults, it can be used alone or in combination with other oral hypoglycemic agents or insulin to control blood sugar. It can also be used as monotherapy or in combination with insulin for children over the age of 10 and adolescents, effectively managing blood sugar across various age groups. Blood sugar control is improved with this medication, aiding diabetes management.

How to take

Adult dose

The dosage should be determined on an individual basis based on the drug's effects and tolerance and must not exceed the maximum recommended daily dose. This medication should be initiated at a low dose to reduce gastrointestinal adverse reactions and to confirm the minimum effective dose necessary for adequate blood sugar control. It should be taken 2 to 3 times daily with 500 mg at each dose along with meals. Dosage increases should be gradual at 500 mg weekly, up to 20.

Child dose

For children over 10 years of age and adolescents, the usual starting dose is 500 mg once daily taken with or after meals. Blood sugar levels should be measured one week later to adjust the dosage.

Precautions

This drug may cause severe lactic acidosis or hypoglycemia, so special care is needed when using it. It should not be administered to patients with moderate to severe impaired kidney function, dehydration, serious infections, or cardiovascular issues. Patients with acute heart failure or those scheduled for tests using iodine contrast agents should also avoid this medication and temporarily discontinue its use before and after the test. This medication should not be given to individuals with hypersensitivity to it, as well as patients with Type 1 diabetes or diabetic ketoacidosis. In cases of severe infections, trauma, or pre and post surgeries, the medication should be temporarily suspended and resumed once the patient's condition stabilizes. Patients with poor nutritional status, impaired liver function, or those who consume excessive alcohol should also take care when using this medication. During administration, there may be an increased risk of lactic acidosis or hypoglycemia due to irregular meals, excessive exercise, or interactions with other medications, so caution is advised. Lactic acidosis is rare but a very serious side effect; symptoms such as fatigue, muscle pain, difficulty breathing, and abdominal pain should prompt immediate medical attention. Gastrointestinal adverse effects are common but mostly transient; taking the medication with meals or gradually increasing the dosage can alleviate these effects. Regular monitoring of kidney function, blood sugar, and vitamin B12 levels is necessary to check for abnormalities, particularly in the elderly or those with impaired kidney function. This medication should not be taken with alcohol due to an increased risk of lactic acidosis, so drinking should be avoided. During pregnancy, it may affect the fetus; thus, it should be administered based on a doctor's judgment, and its use during breastfeeding is not recommended. In cases of overdose, the medication can be removed through blood dialysis, so care should be taken not to exceed the prescribed dosage. This medication aids in blood sugar control without weight gain and should be used at appropriate dosages for children or the elderly.
