Essential hypertension uses medication to normalize blood pressure levels in order to reduce the risk of cardiovascular complications. This medication lowers blood pressure, thereby reducing the strain on the heart and blood vessels and helping to prevent serious complications such as stroke or myocardial infarction. Consistent use can enable blood pressure control and contribute to maintaining healthy blood vessels in the long term. Therefore, it is used for the treatment of essential hypertension.

How to take

Adult dose

The recommended initial dose is 10 mg once daily, taken regardless of meals. If blood pressure is not adequately controlled at this dose, it may be increased to 20 mg once daily. As needed, it can be further increased to a maximum of 40 mg daily. The blood pressure-lowering effect typically appears within 2 weeks of starting treatment, with maximum effect reached in about 8 weeks. For patients at risk for intravascular volume depletion, starting at a lower dose is recommended.

Child dose

Safety and efficacy have not been established for pediatric patients.

Precautions

This medication can cause serious harm to the fetus and newborn if taken during the second and third trimesters of pregnancy, so it should be discontinued immediately if pregnancy is confirmed. It should not be used in patients with allergies to this medication, during pregnancy or breastfeeding, those undergoing dialysis, patients with severe liver dysfunction, or patients with biliary obstruction. Patients with specific genetic issues, such as lactose intolerance, should not take this medication due to the lactose it contains. Caution is required when administering this medication to patients with low blood volume or low sodium levels, as blood pressure may drop too low initially; if hypotension occurs, immediate appropriate action must be taken. Patients with severe heart failure who rely on the renin-angiotensin-aldosterone system may experience a worsening of kidney function after taking this medication; therefore, periodic evaluations are necessary. It should be administered carefully to patients with poor kidney function or kidney transplant patients, and patients with renal artery stenosis also require caution. Because there is a risk of increased potassium levels in the blood during treatment, especially patients with heart or kidney diseases should have regular blood tests. Special caution is necessary when prescribing this medication to patients with aortic or mitral stenosis, or obstructive or hypertrophic myocardial disease. Patients with primary hyperaldosteronism may not respond well to this medication. Due to its blood pressure-lowering effects, dizziness may occur, so avoid driving or hazardous activities. Some ethnic groups may experience a slightly weaker effect on blood pressure reduction with this medication. Although generally safe to use, mild side effects such as dizziness, headache, palpitations, back pain, bronchitis, increased creatine phosphokinase, diarrhea, hematuria, hyperglycemia, hypertriglyceridemia, flu-like symptoms, pharyngitis, rhinitis, sinusitis, cough, abdominal pain, dyspepsia, gastroenteritis, hypercholesterolemia, hyperlipidemia, hyperuricemia, arthralgia, arthritis, myalgia, rash, hair loss, itching, epistaxis, and upper respiratory infections may occur. Rarely, symptoms such as angioedema or liver dysfunction, or allergic reactions may arise; consult a doctor immediately if any abnormal symptoms occur. If chronic diarrhea or severe abdominal pain occurs during treatment, stop taking this medication and consult your doctor. Caution is needed when used in combination with other antihypertensive drugs, potassium-sparing diuretics, or potassium supplements as blood potassium levels may dangerously increase. Avoid co-administration with lithium as it may lead to dangerously elevated lithium levels. Combining this medication with non-steroidal anti-inflammatory drugs may increase the risk of kidney function deterioration, so kidney function should be regularly monitored. Using this medication together with other drugs that have similar actions may increase the risks of hypotension, hyperkalemia, and kidney function deterioration, necessitating careful observation. Not taking this medication during pregnancy is the safest option for the health of the fetus, and breastfeeding mothers should consider stopping the medication or breastfeeding since this drug can be transmitted through breast milk. Safety and efficacy in pediatric populations have not been established. In elderly patients, as the concentration of this medication in the body may increase, dose adjustment and caution are necessary. In cases of overdose, hypotension and palpitations may appear; if symptoms are severe, seek medical attention immediately.
