Precautions

This medication should not be used by women who are pregnant or may become pregnant, or by nursing mothers. Patients with gastrointestinal bleeding, mechanical obstruction, or perforation should not be administered this medication as its gastrointestinal motility-promoting effect can be harmful. Patients with a specific tumor called a chromaffin cell tumor should not use this medication as it may induce hypertension. Those with allergic reactions to this medication or who cannot tolerate it should avoid use. Patients with epilepsy, manic episodes, or manic phases of bipolar disorder should also avoid using this medication. Additionally, this medication should not be administered to patients with malignant breast disease as it may cause adverse reactions related to the breast. This medication contains lactose, so patients with lactose intolerance or related genetic conditions should not use it. Patients with hypertension, children, and elderly individuals should use this medication with special caution. Children may experience extrapyramidal symptoms easily, so care should be taken to avoid excessive dosing, and the elderly may be at risk of prolonged drug presence in the body due to decreased renal function, so attention to adverse reactions is necessary. During the use of this medication, endocrine abnormal symptoms such as amenorrhea, galactorrhea, and gynecomastia may occur, so symptoms should be monitored closely. Extrapyramidal symptoms such as tremors, tongue twisting, and anxiety may rarely occur; if such symptoms appear, medication use should be stopped immediately. Long-term use may result in tardive dyskinesia, characterized by involuntary movements of the face or around the mouth. Gastrointestinal side effects may include dry mouth, heartburn, nausea, vomiting, diarrhea, and constipation, while cardiovascular issues such as hypertension or arrhythmias may arise. This medication can rarely cause a serious side effect known as neuroleptic malignant syndrome, which presents with high fever, muscle rigidity, and altered consciousness; if these symptoms occur, medication use should be stopped immediately, and emergency treatment should be sought. Skin rashes or swelling may also occur rarely, and if such symptoms develop, discontinuation of the medication is necessary. Other reported symptoms include flushing, fatigue, insomnia, drowsiness, dizziness, staggering, erectile dysfunction, and changes in libido. Patients taking this medication are advised to avoid activities that require attention, such as driving a car or operating machinery. There is a risk of cardiac side effects, so patients with heart disease should take particular caution, and those with risk factors for stroke should use this medication cautiously. When using this medication with other drugs, pay attention to interactions. Particularly, concomitant use with anticholinergics, narcotic analgesics, digitalis preparations, or other benzamide medications may increase side effects, so use cautiously under medical guidance. Avoid taking with alcohol, and special monitoring is required when combining with drugs that pose a risk of cardiac arrhythmias. It is safest to avoid combining with drugs that can cause electrolyte imbalances. In principle, this medication should not be used during pregnancy or breastfeeding due to insufficient safety established for the fetus and newborn. Elderly patients should be cautious about dosage adjustment and monitoring for side effects due to potential drug accumulation from decreased renal function. Lastly, this medication has an antiemetic effect that may mask vomiting symptoms caused by other conditions, so thorough diagnosis and observation prior to treatment are necessary. Animal studies have reported decreased reproductive function and potential tumorigenesis; thus, caution is needed with long-term use.
