Improvement of hypercalcemia and electrolyte imbalance (hyperkalemia, hypermagnesemia)
This medication is used as a dialysis perfusion solution for patients with chronic kidney failure. It is particularly helpful when hypercalcemia occurs due to the administration of active vitamin D3 preparations or calcium preparations. Severe alkalosis can occur with dialysis fluid that has a high bicarbonate concentration, but this can be used safely in such cases. It is also suitable for diabetic patients using low-glucose dialysis solutions or patients exhibiting high blood glucose levels initially during dialysis. It is effective even when the improvement of hyperkalemia and hypermagnesemia is insufficient in dialysis solutions with high potassium or magnesium levels. Therefore, this medication is used to adjust electrolyte and acid-base balance in the blood under various circumstances, improving the patient's condition.
An appropriate basic solution for artificial dialysis should be selected based on the patient's condition and the type of artificial kidney used, and this solution should be diluted with this liquid and purified water for use.
This drug is used for dialysis and should be used cautiously in specific patients. Particularly, caution is needed for patients with severe liver dysfunction or severe diabetes, where metabolic acidosis may occur, as symptoms of hypotension may present. Additionally, for patients taking digitalis preparations, low serum potassium levels may increase the risk of digitalis toxicity, so care is required. When using this drug, hyperglycemia may occur, so it is necessary to monitor blood glucose levels carefully during and after dialysis and take measures, such as switching to glucose-free dialysis solutions, if necessary. Weight gain or a tendency for increased blood pressure may occur, potentially due to increased fluid intake from thirst, so fluid removal and management are necessary to manage weight and blood pressure. Symptoms such as cramps or discomfort may occur due to hypocalcemia, so appropriate treatment, including switching to dialysis solutions with higher calcium concentrations or administering calcium preparations, is required. Osteocomplications such as osteoporosis and osteomalacia may arise, necessitating management such as administering active vitamin D3 preparations. Ectopic calcification may occur, and to maintain serum phosphorus levels within the normal range, phosphate binders may be used. Symptoms of dialysis disequilibrium syndrome, such as headaches, nausea, vomiting, and cramps, may arise, requiring measures to adjust dialysis efficiency. It may affect the circulatory system, potentially leading to symptoms of hypotension or shock, necessitating the discontinuation of dialysis or treatment with fluids and blood pressure-raising agents if such symptoms occur. For pregnant women, the safety of this drug has not been established, and it should only be used when the therapeutic benefits outweigh the risks. Caution is needed for children as safety has not been sufficiently verified. Elderly patients are at risk for osteocomplications, recommending regular check-ups and administration of vitamin D3 preparations if necessary. Thoroughly purified water must be used for dialysis dilution, with awareness of potential membrane rupture or bleeding tendencies. When preparing the drug, the prescribed diluent should be used, as concentrations that are too high or low can present symptoms such as headache, blood pressure changes, or altered consciousness, so caution is needed. The electrolyte concentration and pH of the dialysis solution must be confirmed before use, and it must be used immediately after mixing. This drug is not to be used for injections or peritoneal perfusion, and dialysis solutions should be maintained at body temperature for safety.
