Treatment of edema and sodium retention symptoms due to congestive heart failure, liver cirrhosis, and nephrotic syndrome; improvement of essential hypertension.
This medication is primarily used to treat edema caused by various conditions such as congestive heart failure, liver cirrhosis, and nephrotic syndrome. It is particularly beneficial when other treatment methods are ineffective or inappropriate. Additionally, it can be used in cases of essential hypertension that are difficult to control with other treatments. This medicine helps to reduce the excessive accumulation of sodium and fluid in the body, improving edema symptoms and assisting in blood pressure regulation. It is used to effectively alleviate the patient's symptoms and improve their condition.
1. Edema (congestive heart failure, liver cirrhosis, nephrotic syndrome): Adults should take 100 mg of spironolactone and 100 mg of hydrochlorothiazide once or in divided doses orally per day. Depending on the response to the initial dose, each may be adjusted to 25-200 mg. 2. Essential hypertension: Adults should take 50-100 mg of spironolactone and 50-100 mg of hydrochlorothiazide once or in divided doses orally per day. Adjustments may be made based on age and symptoms.
The maintenance dose for children is 1.65-3.3 mg per kg of body weight per day.
This medication contains spironolactone, which has been associated with tumor formation in animal studies; therefore, it should only be used for indicated conditions, and unnecessary use should be avoided. Adjustments are needed depending on the patient's condition when treating hypertension and edema; thus, it is not suitable for initial treatment. Concurrent use with potassium-rich medications or foods may dangerously elevate potassium levels; therefore, co-administration should be avoided. In patients with poor liver function, there is a risk of coma due to fluid and electrolyte imbalance; thus, careful administration is required. Concurrent use with lithium preparations may decrease lithium excretion and increase toxicity, so co-administration should be avoided. Calcium channel blockers may enhance the effects of other antihypertensive medications and may provoke allergic reactions in patients with a history of allergies or asthma. Additionally, some sulfonamide derivatives may exacerbate systemic lupus erythematosus, thus caution is advised. This medication should not be administered to patients with anuria, acute renal failure or severe renal impairment, hyperkalemia, acute or severe liver failure, those taking potassium-sparing diuretics, or those with a hypersensitivity to sulfonamide drugs. This medication contains lactose and should be avoided in patients with lactose intolerance. Patients taking this drug may have an increased risk of non-melanoma skin cancer; therefore, they should minimize sun exposure and regularly check their skin condition. Patients allergic to the yellow No. 5 ingredient in this drug should also exercise caution. This medication may cause various side effects, including dizziness, hypotension, gastrointestinal issues, blood abnormalities, allergic reactions, electrolyte imbalances, muscle cramps, neuropsychiatric symptoms, etc. Special attention should be given to patients with renal function abnormalities or liver dysfunction. During treatment, electrolyte levels in the blood should be regularly monitored to check for abnormalities, and if symptoms of hyperkalemia arise, medical advice should be sought immediately. Pregnant or breastfeeding women should consult a doctor regarding the use of this medication, as it may affect the fetus or newborn. Safety in children has not been established, thus caution is needed when prescribing this drug. In case of overdose, emergency measures such as inducing vomiting or gastric lavage are necessary, and since electrolyte imbalances and central nervous system depression may occur, immediate medical attention should be sought. This drug may interact with other medications, so it is essential to inform the healthcare provider of any other medications being taken, especially when taking digoxin, loop diuretics, corticosteroids, or alcohol.
Hives, severe swelling (edema), chest tightness, difficulty breathing, cold sweat, paleness, severe allergic reactions, high fever with severe rash, blisters, skin peeling, pain in oral/ocular mucosa, severe skin reactions, shortness of breath or difficulty breathing, asthma attacks, coughing with fever along with respiratory issues, yellowing of the skin or eyes (jaundice), liver abnormalities, sudden and severe headaches, thunderclap headaches, seizures, loss of consciousness.

This medication should be stored in a tightly closed container to prevent air or moisture from entering. It is recommended to keep it at a temperature between 1 and 30 degrees Celsius. Proper storage will help maintain the effectiveness of the medication over time.
