This drug is a basic solution for artificial kidney dialysis perfusion fluid used in patients with chronic renal failure. It is particularly helpful in treating hypercalcemia caused by the administration of active form vitamin D3 preparations or calcium supplements. It is suitable for situations where severe alkalosis may occur from high bicarbonate concentrations in the dialysis fluid, and it is also used in diabetic patients with difficulty in blood sugar control when low glucose dialysis solutions are applied or when high blood sugar levels are observed at the start of dialysis. Additionally, it is effective even when hyperkalemia or hypermagnesemia does not improve well with dialysis fluids that have high potassium and magnesium concentrations. Therefore, this drug improves electrolyte and acid-base imbalance issues that may occur under various dialysis fluid conditions.

How it is given

Adult administration

Select an appropriate acidic solution for artificial kidney dialysis based on the patient's condition and the type of artificial kidney used, then dilute it with this solution and purified water.

Precautions

This drug should be used with caution in patients with severely impaired liver function or severe diabetes. This drug may suppress heart function or dilate blood vessels, lowering blood pressure, thus requiring careful administration in such patients. Also, patients taking the heart medication digoxin should be careful as low potassium levels can increase the risk of drug toxicity. This drug may cause blood sugar levels to rise; therefore, blood glucose levels should be closely monitored during or after dialysis, and appropriate measures should be taken if necessary. There may be a tendency for weight gain or an increase in blood pressure, so fluid intake due to thirst should be managed, along with weight and blood pressure checks. If calcium levels are low, there may be symptoms such as cramps or discomfort, and it may be necessary to switch to a dialysis fluid with higher calcium concentrations or administer calcium supplements. Bone health should be managed with the administration of vitamin D preparations as there is a risk of bone disorders such as osteoporosis or osteomalacia. Phosphate binders may need to be used to maintain blood phosphate levels within the normal range. Symptoms of dialysis disequilibrium syndrome such as headache, nausea, vomiting, cramps, and altered consciousness may occur, and dialysis intensity should be adjusted if symptoms occur. If blood circulation rapidly decreases, symptoms of low blood pressure or shock may develop, and in such cases, dialysis should be discontinued or adjusted, and necessary medications should be administered. This drug is used in hemodialysis for patients with chronic renal failure and is included in dialysis fluid to maintain acid-base balance with sodium bicarbonate. If the concentration and composition of the dialysis fluid are not accurately matched, side effects such as headache, palpitations, abnormal blood pressure, and altered consciousness may occur, so caution is necessary. When preparing the dialysis fluid, the established methods and concentrations must be strictly adhered to, and it is important to check electrolyte concentrations and pH before use. Pregnant women or women with the potential for pregnancy should be administered this drug only when the benefits of treatment outweigh the risks. Its safety has not been established in children, so care should be taken in their use. Elderly patients are at risk of bone complications, so regular examinations and necessary treatments should be conducted in conjunction. The dilution water for dialysis must be using water that has undergone a proper purification process, and the condition of the dialysis membrane and the risk of bleeding should also be carefully monitored. This drug is not used for injection or peritoneal dialysis, and the prepared dialysis fluid should be used immediately. The temperature for use should be maintained at approximately body temperature, and it should not be used if insoluble foreign substances are present.
