Precautions
This medication may pose a risk of severe respiratory depression; therefore, it should not be used in obese patients under 18 years of age or in patients with obstructive sleep apnea syndrome or severe lung disease. Additionally, it should be used with caution in combination with central nervous system depressants such as narcotics, benzodiazepines, and alcohol, as this may lead to severe sedation, respiratory depression, coma, or even death. This medication should never be administered to patients with hypersensitivity to it, or those with severe respiratory depression, paralytic ileus, increased intracranial pressure, acute alcohol intoxication, or persistent asthma attacks. Patients with lactose intolerance, acute or chronic pancreatitis, or those on MAO inhibitors should also not use this medication. Particular caution is advised for asthma patients, the elderly, those with hypothyroidism, weakened liver or kidney function, a history of drug dependence or alcohol addiction, gallbladder disease or benign prostatic hyperplasia. Similarly, patients with adrenal function impairment, constipation, or pulmonary heart disease should be monitored closely. This medication may lead to dependence when taken for extended periods; therefore, the dosage should be gradually reduced upon discontinuation, and withdrawal symptoms should be carefully observed. Respiratory depression symptoms may occur, so respiratory status should be closely monitored and immediate appropriate treatment should be provided in case of any unusual reactions. Various side effects such as constipation, nausea, vomiting, headache, drowsiness, dizziness, hallucinations, rash, urinary disturbances, and more may occur, so attention is warranted. Driving or operating dangerous machinery should be avoided due to potential drowsiness or dizziness. This medication may exacerbate side effects when taken with other central nervous system depressants, thus dosages should be adjusted and patient status should be closely monitored when taken in combination. Women who are pregnant or may become pregnant should avoid this medication, and extra caution is required around childbirth due to risks of respiratory depression or withdrawal symptoms in neonates. In cases of nursing, discontinuation of breastfeeding is recommended. Elderly patients should receive careful dosage adjustments due to reduced bodily functions and increased risk of respiratory depression. In cases of overdose, it is crucial to seek immediate medical attention for proper emergency care including inducing vomiting, gastric lavage, and administering activated charcoal. If respiratory depression or reduced consciousness occurs, swift treatment such as airway management, oxygen administration, and assisted ventilation is necessary, and continuous monitoring is required even after narcotic antagonist administration. In cases of severe hypotension or cardiac dysfunction, appropriate intravenous fluid therapy and cardiac treatment should be initiated.
