Summary

Main use
Relief of severe chronic pain
Who takes it
Adults
Adult dose
Take this drug twice daily at 12-hour intervals, swallowing it whole without chewing. The dose is determined according to the severity of pain and the patient's previous analgesic requirements. If the patient reports severe pain, treatment is usually started with 10∼20 mg of this drug per dose, twice daily. If the pain worsens or tolerance to morphine increases, the morphine dose should be increased further. When switching to this drug from another oral morphine preparation, the total daily morphine dose should be equivalent to the previous daily dose, divided in half and given in the morning and evening. When this drug is given in place of an injectable morphine preparation previously administered to the patient, increase the morphine dose sufficiently (by approximately 50∼100 %) to compensate for the reduced analgesic effect of morphine following oral administration, and adjust the dose for each individual patient. As this drug is an extended-release formulation, take care regarding the prolonged duration of action and total dose of morphine associated with it, and administer supplemental injectable morphine if necessary. As with other oral morphine preparations, use with caution in patients with acute abdominal conditions and after abdominal surgery.

How to take

Precautions

This medication contains ingredients that suppress the central nervous system, and taking it with alcohol or benzodiazepines can cause severe drowsiness, respiratory distress, decreased consciousness, and even death. This medication should not be administered to patients with allergies to it, those experiencing severe respiratory distress, persistent asthma attacks, or patients in a state of severe central nervous system suppression. Patients with severely impaired liver or kidney function, those with weak cardiac function, and patients with very low blood pressure or in shock should be particularly cautious when using this medication. Long-term use of this medication may lead to tolerance or dependence, and sudden discontinuation can lead to withdrawal symptoms; therefore, dosage should be gradually reduced according to a physician’s instructions. Drowsiness or dizziness may occur while taking this medication, so driving or operating hazardous machinery should be avoided. This medication should not be used by pregnant or breastfeeding women, and especially women of childbearing potential should consult a doctor before taking it. Elderly patients have decreased physiological functions, so respiratory suppression can easily occur; therefore, careful administration starting from a lower dose is necessary. In case of overdose, breathing may stop or consciousness may be lost, so immediate emergency care is needed and the patient should visit a hospital, receiving narcotic antagonists if necessary. This medication is in an extended-release form, and tablets should be swallowed whole without crushing or chewing; crushing them can lead to rapid release of the medication and serious side effects may occur. Additionally, drug interactions may occur when taken with other medications, so if the patient is currently on any other medications, it is essential to inform their doctor or pharmacist.
