Used for patients with chronic renal failure who have difficulty controlling blood glucose levels or whose previous dialysis solutions have been insufficient for correcting hyperkalemia or hypermagnesemia, or pose a risk of hypercalcemia. This dialysate helps manage blood glucose more effectively and aids in maintaining electrolyte balance. Therefore, it is used to more stably manage the patient’s condition in such special circumstances.

How it is given

Adult administration

Select the appropriate alkaline solution for artificial kidney dialysis according to the patient's condition and the type of artificial kidney used, and dilute it with this solution and purified water for use.

Precautions

This medication should be used with caution in patients with severe liver impairment or severe diabetes. The reason is that acetate may not be metabolized properly, leading to symptoms such as hypotension. Patients on digitalis glycosides should be monitored as serum potassium levels may drop, resulting in symptoms of digitalis toxicity. Hypoglycemia or hyperglycemia may occur during or after dialysis, so attention should be paid to blood glucose changes, and immediate appropriate treatment should be received if symptoms arise. Increased fluid intake due to thirst may lead to weight gain and elevated blood pressure, so fluid removal or weight control may be necessary if needed. Complications such as osteoporosis, osteomalacia, and fibrous osteitis may occur, requiring management such as administration of active forms of vitamin D3. Because serum phosphate levels may be elevated leading to ectopic calcification, phosphate binders should be administered to maintain serum phosphate levels within normal ranges. Symptoms of dialysis disequilibrium syndrome, such as headache, nausea, vomiting, and seizures, may occur; if these symptoms arise, measures such as reducing dialysis efficiency should be taken. It may affect the cardiovascular system, potentially causing hypotension or shock, so if symptoms occur, dialysis should be discontinued, or IV fluids and blood pressure boosters may be necessary. The safety of this medication during pregnancy has not been established, and it should only be used when the benefits outweigh the risks. Safety has also not been established for pediatric patients, so caution is necessary. Elderly patients may have a risk of bone complications, requiring regular clinical tests and administration of active vitamin D3 if necessary. The dilution water for dialysis must use rigorously purified water, and caution should be taken regarding dialysis membrane damage or bleeding tendencies. During preparation, do not directly mix the electrolyte solution for artificial kidney dialysis with sodium bicarbonate solution; use the prescribed diluent instead. If the concentration of the dialysis solution is too high or too low, symptoms such as headaches, blood pressure changes, and altered consciousness may appear, so the concentration must be accurately adjusted. Always verify the electrolyte concentration and pH of the dialysis solution before use; it should be used immediately after dilution and should not be stored at 38 degrees Celsius for more than 2 hours. This medication must not be used for injection or peritoneal dialysis, and it should be used at temperatures close to body temperature during dialysis.
