In life-threatening infections, it may be given up to 5mg (potency) per kg of body weight divided into 3-4 doses, and once symptoms improve, it should be reduced to 3mg (potency) per kg of body weight. Do not exceed 5mg (potency) per kg of body weight without serum concentration measurement.
Precautions
This drug is an aminoglycoside antibiotic primarily used for the treatment of infections. This drug should not be used in certain patients. For example, individuals who are allergic to this drug, other aminoglycoside antibiotics, or bacitracin should not be administered this medication. Additionally, patients with inner or middle ear disease, severe renal failure, pregnant or breastfeeding women, patients with muscle disorders, patients with bronchial asthma, and patients with eighth cranial nerve disorders should not use this drug. Patients with impaired kidney or liver function, the elderly, premature infants or newborns, and patients with poor nutritional status should be treated with extra caution. This drug can occasionally cause neurological adverse reactions such as dizziness, tinnitus, and hearing loss, and rarely kidney dysfunction, liver abnormalities, blood disorders, and skin rashes may appear, so symptoms should be carefully monitored. During treatment, serum drug levels should be regularly measured to ensure they remain within an acceptable range, especially in patients with impaired kidney function or the elderly. This drug may have an increased risk of side effects when used with other nephrotoxic medications or muscle relaxants and anesthetics, so caution is needed when co-administering. During pregnancy, it may cause neurological damage to the newborn and should only be used if absolutely necessary, and breastfeeding should be stopped during treatment. When used in the form of eye drops or eye ointment, symptoms such as eyelid itching, swelling, redness, and temporary stinging of the eyes may occur, and use should be discontinued if these occur. When using this drug, it is preferable to perform sensitivity testing to prevent the emergence of bacterial resistance and to use it for the minimum necessary duration. Additionally, during treatment, avoid using contact lenses, and avoid driving or operating machinery as vision may become blurred. In case of overdose, severe side effects such as kidney dysfunction, hearing loss, and respiratory paralysis may occur, requiring immediate medical attention. This drug should not be mixed with beta-lactam antibiotics when used as an injection, and care should be taken not to inject eye drops or ointments into the eye. Finally, this drug has reported resistance against certain bacteria, so it must be used exactly according to medical professionals' instructions.
