This medication is used in various forms, and its effectiveness varies according to each indication. The liquid form of the 2% viscous preparation is used in otorhinolaryngology for local anesthesia in cases of irritation and inflammation of the oral and pharyngeal mucosa and is used in dentistry to suppress gag reflex during X-ray imaging or dental impressions. The 2% gel formulation provides surface anesthesia and lubrication, used during cystoscopy, catheter insertion, urethral surgeries, endoscopy, proctoscopy, and intubation, helping to relieve pain from cystitis or urethritis. The injection formulation varies depending on the concentration and use, with the 0.5% intramuscular injection relieving pain during antibiotic injections. The 1% and 2% injections are used for epidural anesthesia, nerve block, infiltration anesthesia, surface anesthesia, and also in the treatment of ventricular arrhythmias, while the 0.5% injection is also used for anesthesia purposes. The 4% injection is used for surface anesthesia and can be diluted for various anesthesia types. Thus, this medication is used for various anesthesia and pain relief applications.

Precautions

This medication should not be used in patients with severe bleeding or shock, patients with inflammation at the injection site or sepsis, and patients who are allergic to this medication or other amide-type local anesthetics. Furthermore, a serious side effect called malignant hyperthermia can rarely occur, so if there is a sudden increase in body temperature, muscle rigidity, or blood discoloration, administration should be stopped immediately, and professional treatment should be sought. Central nervous system symptoms may include tremors, seizures, drowsiness, anxiety, dizziness, and nausea, which should be promptly treated if they occur. This medication can cause allergic reactions like skin rashes or swelling and, in severe cases, anaphylaxis, so any unusual symptoms should be reported to the healthcare provider immediately. During the use of local anesthetics, sudden shock or symptoms of poisoning can occur, so preparations for emergency treatment are always necessary. Careful observation of the patient's general condition is required during administration, using the lowest effective concentration and minimum dose possible, particularly being cautious when used in conjunction with vasoconstrictors. Especially when injected in highly vascular areas, the absorption is rapid, so it is recommended to use only a small volume. Ensure that the injection needle does not enter blood vessels or spinal fluid, and it is advisable to administer slowly if possible. This medication should not be used in conjunction with certain vasoconstrictors, and caution is necessary when combined with some drugs, as it may lead to cardiac function suppression or increased toxicity. The safety of this medication in pregnant women has not been established, so it should only be used when the benefits of treatment outweigh the risks. Elderly individuals may be more sensitive to the effects of the drug, so dosage adjustments and monitoring of the patient's condition are important. Lastly, the metabolites of this medication cannot be excluded from the possibility of causing mutations or tumor development with prolonged high-dose use, so it is advisable not to administer at high concentrations for extended periods.
