Summary

Main use
Prevention of cardiac arrest and myocardial protection effect
Who uses it
Adults
Adult dosage
This liquid must be used after adding sodium bicarbonate (10mL of 8.4% sodium bicarbonate to 1000mL of this liquid) within 24 hours. The solution should be cooled to 4°C and immediately perfused into the aortic root using the coronary perfusion motor pump of the heart-lung machine at the time of aortic occlusion. Simultaneously with perfusion, normothermic physiological saline or Ringer's solution that has been cooled to 4°C should be injected into the thoracic cavity to induce local cooling around the heart. The local cooling solution should be appropriately maintained.

Efficacy & effects

Prevention of cardiac arrest and myocardial protection effect

View approved original text

This perfusion solution is used to prevent cardiac arrest that may occur during open-heart surgery and to protect the myocardial. It helps to maintain heart function during surgery and minimizes damage. Therefore, it plays an important role in supporting the safe and effective progress of open-heart surgery and aiding in patient recovery.

Dosage and Administration

Precautions

This medication must be combined with sodium bicarbonate before use. Caution is required when perfusing this medication in large volumes, as high levels of magnesium and potassium in the blood can occur if it is not removed by direct suction from the right atrium or right ventricle. When administered to patients undergoing open-heart surgery, there is a potential for cardiac-related issues such as myocardial infarction, ECG abnormalities, arrhythmias, and ventricular fibrillation. Additionally, overdose of this medication may cause tissue edema, so dosage adjustment should be monitored. This medication must be administered by a physician experienced in open-heart surgery, and it is important to continuously monitor the heart temperature to maintain a state of hypothermia during surgery. When used, this medication should be injected only into the cardiovascular system, and intravenous injection is strictly prohibited. Furthermore, it should only be used as a perfusion solution in cases of aortic occlusion, and it should not be used if the solution is cloudy or if the container is damaged. Any remaining solution should not be reused and should be discarded.
