Relief of severe and moderate acute/chronic pain (such as pain from cancer, diagnosis, and postoperative pain)
This medication is used to relieve severe and moderate acute and chronic pain, such as pain associated with cancer or pain that occurs after diagnosis and surgery.
As tramadol hydrochloride, administer 50-100 mg by intravenous or intramuscular injection, and repeat every 4-5 hours as necessary. The maximum daily dose can be 400 mg.
This medication is in injectable form, and glass shards may mix in when cutting the container, so caution is essential, especially when used by children or the elderly. Administering the drug in higher than recommended doses may cause seizures, and it should be used with caution only when necessary in patients at risk for seizures. Long-term use can lead to psychological and physical dependence; therefore, it should be administered under strict supervision for a short duration in patients with a history of substance abuse or dependence. Using this medication alongside alcohol or central nervous system depressants can lead to severe drowsiness, respiratory depression, coma, and in severe cases, death, thus extreme caution is required when combined. Patients with specific genetic characteristics (CYP2D6 ultra-rapid metabolizers) may rapidly generate active metabolites of this drug, which can increase the risk of respiratory depression, so dose adjustment and symptom monitoring are necessary. This medication should not be administered to children under 12 years or to children under 18 years who have undergone tonsillectomy or adenoidectomy; some children aged between 12 and 18 should also avoid it if they have risk factors. This medication should not be used in patients with acute alcohol intoxication, central nervous system intoxication, severe respiratory depression, head injuries, patients taking MAO inhibitors, severe kidney and liver impairment, or in pregnant and nursing women. When administering this medication, caution is advised in patients simultaneously using morphine, those with liver impairment, those who are drinking, those with kidney impairment, or those with a history of seizures, especially in patients with a history of substance abuse or mental illness where the risk of dependence and abuse is high. Patients may experience drowsiness or dizziness during treatment, so they should avoid driving or operating dangerous machinery, and it is not advisable for use in minor pain. This medication can interact with various drugs; hence, when taken with benzodiazepines, other narcotic analgesics, antidepressants, antipsychotics, or alcohol, careful observation and dose adjustment are necessary. Pregnant or nursing women should not use this medication, particularly nursing mothers should be cautious as this medication and its metabolites may pass into breast milk, posing risks to the infant. In case of overdose, severe respiratory depression and seizures may occur, necessitating immediate emergency care, with naloxone being a possible antidote. This medication must be stored out of reach of children and should not be kept in another container outside of its original packaging.
