Treatment of epilepsy (e.g., tonic-clonic and partial seizures) and digitalis-induced arrhythmias
This medication is effectively used for various types of seizures in patients with epilepsy. It aids in the treatment of tonic-clonic seizures (grand mal), partial seizures (focal seizures), and psychomotor seizures when given in the form of tablets or capsules. The injectable form is particularly used for status epilepticus and situations where oral administration is difficult, such as during consciousness impairment or during and after surgery to suppress seizures. It is also used for digoxin-induced heart failure when first-line treatments are ineffective. This medication effectively controls various seizures and improves symptoms.
The dose should be adjusted according to the severity of seizures and the patient's tolerance; measure serum concentrations if necessary. This drug must not be administered once daily and must be given in divided doses. 1. Adults: Administer 100 mg of phenytoin orally 3 times daily; the maintenance dose is 300 ~ 400 mg of this drug in divided doses. The dose may be increased up to 600 mg daily. The dose may be adjusted appropriately according to age and symptoms. (Injection) 1. Status epilepticus or seizures that cannot be controlled with oral administration Adults: Slowly administer 125 ~ 250 mg of phenytoin sodium by intravenous injection at a rate not exceeding 50 mg/min. If seizures cannot be controlled with the above dose, administer an additional 100 ~ 150 mg of this drug after 30 minutes or consider other treatment. When convulsions have ceased and consciousness has returned following administration of this drug, switch to oral administration. 2. Digoxin-induced arrhythmias for which first-line treatment is ineffective Adults: Administer an initial dose of 3.5 ~ 5 mg/kg of body weight of phenytoin sodium by intravenous injection. Repeat once if necessary. This drug should be administered slowly by intravenous injection at a steady rate not exceeding 50 mg/min. The dose may be adjusted appropriately according to age and symptoms.
Children: Administer 5 mg/kg of body weight of phenytoin in 2 ~ 3 divided doses; the maintenance dose is 4 ~ 8 mg/kg of body weight daily of this drug in divided doses. The daily dose may be increased up to 300 mg.
