This medication is used for various endocrine disorders, primarily and secondarily adrenal cortical insufficiency, and adrenal crisis. It is also used as a short-term adjunct therapy to prevent acute exacerbations in rheumatic disorders such as rheumatoid arthritis, osteoarthritis, and acute gouty arthritis. It is applicable in connective tissue diseases like systemic lupus erythematosus, acute rheumatic myocarditis, and dermatomyositis, as well as various skin disorders like pemphigus, psoriasis, and neurodermatitis to alleviate symptoms. For allergic diseases, it is used in the treatment of bronchial asthma, atopic dermatitis, allergic rhinitis, and anaphylactic shock. Moreover, it is effective in improving symptoms and treating various conditions such as ophthalmic diseases, ulcerative colitis, acquired hemolytic anemia, and nephrotic syndrome. In this way, it is used to improve patient conditions by reducing inflammation and modulating immunity in several diseases.

How to take

Adult dose

In adults, the usual dosage of dexamethasone is 0.5-8mg once daily, divided into 1-4 doses. Adjust according to age and symptoms.

Child dose

In children, administer 0.15-4mg once daily, divided into 1-4 doses. Adjust according to age and symptoms.

Precautions

This medication should not be used in specific patients. For instance, patients who are allergic to this drug or its components, those who have received live vaccines, individuals with herpes simplex, herpes zoster, chickenpox, and patients with infections without appropriate antimicrobial treatment or systemic fungal infections should not receive this medication. Additionally, this medication contains lactose and should not be used in patients with lactose intolerance or certain genetic metabolic disorders. There are also patients for whom this medication should be administered with caution. For example, special care should be taken with patients who have a history or related symptoms of glaucoma, tuberculosis, cataracts, hypertension, thrombosis, peptic ulcers, psychosis, osteoporosis, diabetes, renal failure, liver cirrhosis, fatty liver, myasthenia gravis, and epilepsy. When using this medication, it is important to closely monitor for potential worsening of infections. Moreover, various side effects such as adrenal insufficiency, elevated blood sugar, gastrointestinal issues, neurological disturbances, osteoporosis, fluid and electrolyte imbalances, eye diseases, blood disorders, and skin reactions may occur; hence symptoms should be observed carefully. When used for a prolonged period, adrenal insufficiency may occur, and cessation of the medication suddenly can result in severe side effects, so the dosage should be tapered gradually. Additionally, there is an increased risk of infections, so live vaccinations should be avoided, and care should be taken to prevent exposure to infectious diseases. This medication can interact with various drugs; hence it is imperative to confirm interactions with other medications being taken. In particular, caution is required when used in conjunction with drugs that induce or inhibit specific hepatic metabolism enzymes, as this may alter the drug's efficacy. Pregnant or breastfeeding women should only use this medication when the benefits outweigh the risks to the fetus or newborn, and it is recommended to discontinue breastfeeding. When administered to children, the minimum effective dose should be used due to potential side effects such as growth retardation, and growth status should be monitored closely. Elderly patients may be more susceptible to side effects such as infections, diabetes, osteoporosis, hypertension, cataracts, and glaucoma, necessitating careful administration and regular monitoring. Finally, contracting chickenpox or measles while using this medication can lead to severe outcomes, so it is crucial to confirm medical history and vaccination status and take appropriate precautions against infections.

Side effects and when to seek care immediately

Serious side effects

Urticaria, severe swelling (edema), chest tightness, difficulty breathing, cold sweat, pallor, rashes, anaphylaxis-like reactions; severe rash with high fever, blisters, skin peeling, mucosal pain in the mouth/eyes; difficulty breathing or shortness of breath, asthma attacks, respiratory symptoms with cough + fever; yellowing of the skin or eyes (jaundice), extreme lethargy; sudden and severe headache, thunderclap headache, seizures, altered consciousness.

Mild side effects

Onset and exacerbation of infections, secondary adrenal cortical insufficiency, diabetes, menstrual irregularities, Cushing's syndrome, buffalo hump, peptic ulcers, gastrointestinal bleeding, pancreatitis, diarrhea, nausea, vomiting, stomach pain, heartburn, abdominal bloating, thirst, loss of appetite, increased appetite, psychiatric disorders, depression, euphoria, insomnia, headache, dizziness, seizures, osteoporosis, aseptic necrosis of the femur and humeral head, myopathy, myalgia, arthritis.

Not everyone experiences these, and they can vary from person to person.
