Bone grafting during implant placement

Do I always need bone grafting when getting an implant? Or does it depend on my gum condition?

So, is it deciding whether to do a bone graft or not? If you do a bone graft, there will be an additional fee.

I'm wondering if this occurrence might be considered over-treatment.
