According to a study published by a research team from Harvard University in the United States in the journal JAMA Network Open, alcohol consumption can improve cholesterol levels and reduce the risk of cardiovascular disease.

The research team conducted a longitudinal analysis of regular health check-up data over a maximum of one year, targeting approximately 58,000 adults who undergo annual health screenings at the Japan Preventive Medicine Center. The results showed that when non-drinkers started drinking, their 'bad' cholesterol (LDL) levels decreased, and their 'good' cholesterol (HDL) levels tended to increase. In particular, HDL levels increased to a degree that generally surpasses the improvement effects seen in medication treatments.

Conversely, among those who abstained from alcohol during the research period, opposite changes were observed. LDL levels increased and HDL levels decreased after abstinence. These changes in cholesterol levels tended to be proportional to the amount of alcohol consumed.

Specifically, people who started drinking less than 1.5 drinks per day experienced a decrease in LDL cholesterol by 0.85 mg/dL and an increase in HDL cholesterol by 0.58 mg/dL compared to non-drinkers. In the group that consumed 1.5 to 3 drinks, LDL decreased by 4.4 mg/dL and HDL increased by 2.49 mg/dL, while in the group that drank 3 or more drinks per day, LDL decreased by 7.44 mg/dL and HDL increased by 6.12 mg/dL.

Conversely, among people who abstained from alcohol, the group that reduced their intake from more than 1.5 drinks per day to none experienced an increase in LDL by 1.10 mg/dL and a decrease in HDL by 1.25 mg/dL. The group that reduced their intake from 1.5 to 3 drinks per day to abstinence saw an increase in LDL by 3.71 mg/dL and a decrease in HDL by 3.35 mg/dL. The group that reduced their intake from more than 3 drinks per day to abstinence experienced an increase in LDL by 6.53 mg/dL and a decrease in HDL by 5.65 mg/dL.

The optimal LDL level for adults is recommended to be below 100 mg/dL, and HDL should be 60 mg/dL or higher.

If LDL levels are high, the risk of heart disease and stroke increases, and if HDL is high, it has a preventive effect against cardiovascular disease. The researchers explained that although the changes in cholesterol observed in this study may seem small, an increase of 5 mg/dL in LDL raises the risk of cardiovascular disease by 2-3%.

The research team explained that the same results were obtained even after adjusting for various factors, including basic indices such as age, gender, and body mass index of the study subjects, as well as medical conditions like hypertension and diabetes, lifestyle factors such as exercise, eating habits, and smoking. Changes in cholesterol levels showed a consistent trend across all types of alcohol, regardless of the main type, including wine, beer, sake, and other spirits.

The researchers emphasized, "The results of this study do not suggest encouraging abstainers to drink nor do they recommend excessive drinking for those who have previously consumed alcohol."

However, the research team explained, "After quitting alcohol, it is necessary to monitor cholesterol levels more carefully to manage potential cardiovascular disease risks."

According to this study, alcohol consumption affects cholesterol levels and may have some positive effects on cardiovascular disease. However, alcohol also increases the risk of cancer, raises the incidence of hypertension and coronary artery disease, and generally has negative effects on health. In particular, it can cause cirrhosis, increase mortality rates, and elevate the risk of dementia, so caution is necessary.
