Improvement of essential hypertension and heart failure (left ventricular dysfunction)
This medication helps to control the blood pressure of patients with essential hypertension. Additionally, it is used as an adjunct treatment in heart failure patients with impaired left ventricular contraction function who do not achieve adequate effects from ACE inhibitors alone or do not tolerate them well. It contributes to reducing the burden on the heart and improving heart failure symptoms. Therefore, it is effectively used in the treatment of essential hypertension and specific heart failure patients.
For essential hypertension, the recommended initial and maintenance dose is 8 to 16 mg of Candesartan Cilexetil taken orally once daily. If the blood pressure is not adequately controlled after 4 weeks at the 16 mg dose, it can be increased to a maximum of 32 mg taken once daily. In the case of heart failure, the initial dose is recommended to be 4 mg taken orally once daily, and the dose may be doubled at intervals of at least 2 weeks.
The safety and efficacy in children and adolescents under 18 years of age have not been established.
This medication can cause serious harm to the fetus and newborns, especially during the second and third trimesters of pregnancy, so if pregnancy is confirmed, it should be discontinued immediately. Patients with a known allergy to this medication, pregnant or nursing women, patients with severe liver dysfunction or bile duct obstruction, and patients with hereditary angioedema should not use it. Additionally, patients with galactose intolerance, Lapp lactase deficiency, or glucose-galactose malabsorption should also not take this medication. Elderly patients, those with hyperkalemia, liver disease, valvular heart disease, ischemic heart disease, or cerebrovascular disorders should use this medication with caution. Side effects such as dizziness, back pain, and respiratory infections may occur, and in heart failure patients, hypotension, hyperkalemia, and renal dysfunction may also be observed, so caution is required. Regular monitoring of serum potassium and creatinine levels is important to assess kidney function and electrolyte status. When used with other antihypertensive agents, the blood pressure-lowering effect may be enhanced, so dose adjustments and patient monitoring are necessary. Particularly when taken in conjunction with potassium-sparing diuretics or potassium supplements, serum potassium levels may rise to dangerous levels, thus requiring caution. Use during pregnancy poses risks to the fetus, and women of childbearing age or those who are pregnant should consult a physician. Nursing women should preferably avoid this medication, and if it is necessary to use it, breastfeeding should be discontinued. The safety and effectiveness of this medication in children and adolescents under 18 years of age have not been established. Elderly patients are at risk of excessive blood pressure lowering leading to cerebral infarction; therefore, they should exercise caution when using this medication. In cases of overdose, hypotension, dizziness, and tachycardia may occur, necessitating immediate medical attention, and this medication is not removed through dialysis. This medication should be kept out of the reach of children, and transferring it to other containers should be avoided to maintain quality.
