In patients experiencing inadequate blood sugar control with metformin alone or in combination therapy, a recommended dose is Empagliflozin 5 mg twice daily (total daily amount of 10 mg) along with the patient's previous metformin dosage. Precautions

This medication is an antidiabetic agent that helps control blood sugar. However, it may cause a rare but serious side effect called lactic acidosis; therefore, it should not be used in patients with impaired kidney function or those with severe infections, dehydration, etc. Additionally, when taken alongside other antidiabetic medications such as insulin or sulfonylureas, there is a risk of hypoglycemia, so caution is advised. Patients with moderate to severe renal impairment or conditions such as acute myocardial infarction, shock, or heart failure should refrain from using this medication. Prior to and after using iodinated contrast agents for tests, the administration of this medication should be temporarily discontinued and resumed only after kidney function is verified to be normal. Women who are pregnant or may become pregnant, as well as nursing mothers, are advised against using this medication. Regular kidney function and blood tests must be performed during the course of treatment, especially for elderly patients or those with compromised kidney function. This medication increases urine output, which may lead to fluid loss, so patients at risk for dehydration or hypotension should be monitored closely. Furthermore, there is a possibility of developing genital infections or urinary tract infections; hence, immediate consultation with healthcare professionals is required if symptoms arise. Alcohol consumption should be avoided during the course of this medication, and the risk of hypoglycemia may increase following irregular meals or vigorous exercise. In preparation for surgery or during acute illness, it is advisable to temporarily suspend this medication and resume it post-recovery. This medication is not recommended for children, and dosage adjustments should be made according to renal function without exceeding the maximum dose in elderly patients. In cases of overdose, hemodialysis may be necessary, and immediate medical attention should be sought. Finally, this medication should be stored out of the reach of children and retained in its original container for safety.
