This medication is used to improve atherosclerosis-related symptoms in adult patients with ischemic stroke, myocardial infarction, and peripheral arterial disease. Additionally, it alleviates atherosclerotic symptoms such as death due to cardiovascular events, myocardial infarction, and stroke in patients with acute coronary syndrome, particularly in those who have received drug treatment, coronary intervention, or coronary artery bypass surgery for unstable angina or non-Q-wave myocardial infarction. Furthermore, in patients with atrial fibrillation who have one or more vascular risk factors and where vitamin K antagonists are inappropriate and the risk of bleeding is low, it helps to reduce the risk of atherothrombosis and thromboembolism, including stroke. It is thus used effectively in preventing and treating cardiovascular diseases and related thrombotic conditions.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, the recommended dosage is 75 mg orally once daily. For patients with acute coronary syndrome, begin with a loading dose of 300 mg orally on the first day, followed by a maintenance dose of 75 mg once daily. For patients with atrial fibrillation, the recommended dosage is 75 mg orally once daily. This medication can be taken regardless of food intake.

Precautions

This drug is an agent that prevents blood platelets from aggregating to prevent clot formation. Patients with genetically reduced CYP2C19 enzyme function may not activate this drug sufficiently, reducing its efficacy; alternative therapies should be considered for such patients. Coadministration with drugs that induce the CYP2C19 enzyme may increase the risk of bleeding and is not recommended. This medication should not be administered to patients who are allergic to it, have bleeding, severe liver damage, are breastfeeding, have certain genetic disorders, or are allergic to soybean oil, beans, or peanuts. Patients at high risk of bleeding or those scheduled for surgery should be cautious of bleeding symptoms while taking this drug, and should discontinue use for a certain period before surgery. The risk of bleeding may increase when this drug is taken with other medications that increase bleeding risk, so caution is required. During the use of this drug, any symptoms of bleeding or abnormalities in blood should be reported to medical personnel immediately. The risk to the fetus during pregnancy is not high, but there may be an increased risk of bleeding during delivery, necessitating discontinuation of the medication before delivery. If breastfeeding, it should be discussed with healthcare providers whether to discontinue the medication as it may be secreted in breast milk. The safety and efficacy in pediatric populations have not yet been established. Elderly patients or those with impaired renal and hepatic function may have a higher risk of bleeding and should be closely monitored during administration. Overdosage may increase the risk of bleeding, and while there is no specific antidote, the effects may be partially reversed with platelet transfusions. This medication should be stored out of reach of children and should not be transferred to other containers.
