Efficacy & effects

Effectiveness of the alkaline solution of dialysate for hemodialysis treatment in chronic kidney disease

View approved original text

This drug is an alkaline solution of dialysate used for hemodialysis in patients with chronic kidney disease. It helps to remove waste and excess electrolytes from the body in patients with impaired kidney function, regulating the internal environment. This plays an important role in supporting kidney function and maintaining electrolyte and acid-base balance. It is used to enhance the effectiveness of dialysis treatment in chronic kidney disease patients and to sustain a healthy life.

How it is given

Adult administration

Based on the patient's condition and the type of artificial kidney used, an appropriate acidic solution for artificial kidney dialysis should be selected and diluted with this liquid and purified water.

Precautions

This drug contains sodium bicarbonate used in the dialysate for hemodialysis in chronic kidney disease patients. Special caution is needed for patients with severe liver impairment or severe diabetes leading to problems with acetate metabolism. If acetate is not properly metabolized, it can cause heart function decline or low blood pressure. Patients taking digitalis preparations should use this drug carefully as low serum potassium levels may lead to signs of drug toxicity. If serum calcium and phosphate levels are high before dialysis, this drug may induce metastatic calcification, requiring caution. Patients with inadequate caloric intake or those on dietary restrictions for diabetes or using blood glucose-lowering agents may be at risk for hypoglycemia. Symptoms of hypoglycemia may occur during or after dialysis, and appropriate measures such as dextrose injection or sugar supply are necessary in such cases. As serum calcium levels may rise, if hypercalcemia occurs, it is necessary to switch to a low-calcium dialysate. Bone-related complications such as osteoporosis, osteomalacia, and fibrous osteitis may occur, and appropriate treatments such as administration of active vitamin D3 preparations are necessary in such cases. Dialysis disequilibrium syndrome may present symptoms such as headaches, nausea, vomiting, convulsions, or altered consciousness, and if symptoms are present, dialysis efficiency should be adjusted. It can affect the circulatory system, leading to low blood pressure or shock symptoms, so if such symptoms occur, dialysis should be interrupted, efficiency lowered, and fluids and blood pressure-raising agents administered. The usage purpose and cautions vary depending on the presence of glucose; if glucose is absent, it can aid in blood sugar control for patients with hyperglycemia. Dialysis dilution water must use water that has undergone a strict purification process, and caution is warranted regarding any damage to the dialysis membrane or tendency for bleeding. During formulation, it must be prepared exactly with the designated diluent; if the concentration is too high or low, side effects such as headaches, blood pressure changes, or altered consciousness may occur. Electrolyte concentration, osmotic pressure, and pH of the dialysate must be confirmed before use, and it should be used as soon as possible after mixing. Women who are pregnant or may become pregnant should only use this drug if the benefits outweigh the risks. Safety in children is not established, so it should be used cautiously, and elderly patients are at risk for bone complications, necessitating regular check-ups. This drug should not be used for injection or peritoneal dialysis and should be used at a temperature close to body temperature.
