This medication is used to improve symptoms due to atherosclerosis in adult patients with ischemic stroke, myocardial infarction, or peripheral arterial disease. It also helps reduce cardiovascular risk related to atherosclerosis in patients with acute coronary syndrome, specifically those with unstable angina or non-Q-wave myocardial infarction who have undergone drug therapy, coronary intervention, or coronary artery bypass surgery. In cases where patients with atrial fibrillation have one or more vascular risk factors and a low bleeding risk, and where vitamin K antagonists are not appropriate, it is also used to reduce the risk of stroke and thromboembolism. This drug is effective in improving vascular health and reducing the risk of cardiovascular complications.

How to take

Adult dose

1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, take Clopidogrel 75 mg orally once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), start with a loading dose of 300 mg on the first day of treatment and follow with a maintenance dose of 75 mg orally once daily. During this time, aspirin 75 to 325 mg should be taken in conjunction with this medication.

Precautions

This medication is an agent that inhibits platelet aggregation in the blood, preventing thrombus formation. Patients with genetically reduced function of a specific enzyme (CYP2C19) may experience decreased effectiveness of this drug; in such cases, treatment options should be reconsidered or alternative medications should be considered. Moreover, co-administration with drugs that activate certain enzymes may lead to increased concentrations of the active ingredient of this drug, thereby increasing the risk of bleeding; hence, co-administration should be avoided. Patients who are allergic to this medication or its ingredients, those with bleeding disorders, severe liver impairment, lactating women, and those with genetic issues like lactose intolerance should not take this medication. Patients at high risk of bleeding, those with liver or kidney problems, recent stroke, hypertensive patients, the elderly, and underweight individuals should use this medication with caution and seek immediate medical advice if bleeding symptoms occur. This medication prolongs bleeding time; therefore, patients scheduled for surgery should discontinue it 5-7 days prior to the procedure, and caution is advised when used with other drugs that increase bleeding risk. During treatment, if bleeding symptoms or adverse reactions occur, patients should notify their doctor promptly and undergo appropriate tests such as blood tests. This medication can cause various side effects such as gastrointestinal disturbances, rash, headache, dizziness, diarrhea, nausea, high cholesterol, joint pain, back pain, bruising, epistaxis, depression, upper respiratory infections, rhinitis, bronchitis, cough, pruritus, urinary tract infections, chest pain, weight loss, weight gain, flushing, weakness, stiffness, facial swelling, syncope, constipation, vomiting, ulcers, and loss of appetite, most of which are mild or similar to the effects of aspirin. While the risk to the fetus during pregnancy is not known, caution should be exercised due to the increased risk of bleeding during delivery. Lactating women should decide on use after consulting their physician. Safety and efficacy in children have not been established; therefore, it is advisable not to administer this medication to children. Overdose may increase the risk of bleeding; there is no specific antidote, but platelet transfusions can reverse the effects. This medication should be stored out of reach of children and should not be transferred to other containers. Patients taking this medication are not known to be significantly affected when driving or operating machinery, but individual variations may occur; hence, caution is advised.
