Treatment of essential hypertension and heart failure (intolerance to ACE inhibitors), reduction of mortality risk after myocardial infarction
This medication is used to treat essential hypertension. It is also effective in heart failure patients who are intolerant to ACE inhibitors, specifically those classified as NYHA Class II to IV. Additionally, it helps reduce the risk of death in stable patients with left ventricular dysfunction or left ventricular systolic dysfunction after myocardial infarction. Therefore, it is used in the treatment of hypertension, certain types of heart failure, and the prevention of complications after myocardial infarction.
For essential hypertension, the recommended dose is 80 mg orally once daily as Valsartan. If blood pressure is not well controlled, the dose can be increased to 160 mg once daily or combined with diuretics, with a maximum dose of 320 mg once daily. Elderly patients (75 years and older) should start at 40 mg once daily. In patients with renal impairment, if creatinine clearance is 10-20 mL/min, start at 40 mg once daily.
The safety and efficacy in pediatric patients under 18 years of age have not been established.
This medication can cause serious harm to the fetus, especially in the second and third trimesters of pregnancy, so it should be discontinued immediately upon confirmation of pregnancy. Patients who are allergic to this medication or its components, pregnant or breastfeeding women, patients with severe liver or kidney disease, and individuals with a history of hereditary angioedema should not take it. Caution should be taken when using this medication in the elderly, patients with high serum potassium levels, patients with liver impairment, and those with heart disease, as hypotension may occur. It is advisable to start with a low dose and gradually increase it. As this medication lowers blood pressure, it may cause dizziness or fainting if blood pressure drops suddenly, so patients should consult their doctor immediately if these symptoms occur. When taken with other antihypertensive medications, diuretics, or potassium-sparing diuretics, blood potassium levels and kidney function should be periodically monitored, and the blood pressure control effect may be reduced when used with certain anti-inflammatory analgesics. This medication is not recommended during breastfeeding, and if it must be taken, it is advisable to stop breastfeeding. Safety and efficacy have not been established in children or adolescents under the age of 18, and its use is not recommended. In case of overdose, severe hypotension and decreased consciousness may occur, and it is essential to visit a medical facility immediately for appropriate treatment.
