Summary

Main use
Treatment of essential hypertension and heart failure (intolerance to ACE inhibitors), reduction of the risk of death after myocardial infarction
Who takes it
Adults · Children
Adult dose
1. Essential Hypertension - Take with water during meals or between meals. - It is recommended to take 80 mg orally once daily at the same time each day (e.g., in the morning) if possible. - For patients with insufficient blood pressure control, the dose may be increased to 160 mg per day or diuretics may be used in combination, and the dose may be increased up to a maximum of 320 mg. - Elderly patients (75 years and older), patients with renal impairment, and patients with hypovolemia should 2. Heart Failure Adults: The starting dose is recommended as 40 mg orally twice a day, which may be increased to 80 mg or 160 mg per dose. When using diuretics, consider reducing the diuretic dose. The maximum dose is 320 mg per day, divided doses. This medication is not recommended to be combined with ACE inhibitors and beta-blockers in triple therapy. 3. Reduction of Risk of Death After Myocardial Infarction Adults: Treatment can be initiated within 12 hours after the occurrence of myocardial infarction. The starting dose is recommended as 20 mg orally, twice a day, and subsequently increase the dose to 40 mg, 80 mg, or 160 mg twice daily, considering the patient’s tolerance over several weeks. Consider dose reduction if symptoms of hypotension or renal failure occur. This medication can also be administered to patients under other treatment regimens (e.g., thrombolytics, acetylsalicylic acid, beta-blockers, and statins) after myocardial infarction. However, combination with ACE inhibitors is not recommended. 4. Patients with Renal Impairment: No dose adjustment is necessary if creatinine clearance is greater than 10 mL/min, but there is no experience in the use for those with creatinine clearance less than 10 mL/min or in dialysis patients, thus this medication should not be used. 5. Patients with Liver Impairment: For mild to moderate liver impairment, it is advisable not to exceed a daily dose of 80 mg. If doses exceeding 80 mg are administered, the benefit of increasing the dose should outweigh the risk. This medication should not be administered to those with severe liver impairment, cirrhosis, or cholestasis. 6. Pediatric Patients: Safety and efficacy have not been established in pediatric patients and adolescents under 18.
Child dose
Safety and efficacy have not been established in pediatric patients and adolescents under 18, so use is not recommended.

View approved original text

This medication is used to reduce the risk of death in patients with essential hypertension, heart failure (especially NYHA class II to IV heart failure patients who cannot use ACE inhibitors), and left ventricular dysfunction after myocardial infarction. Therefore, it is effective in controlling blood pressure, supporting heart function, and preventing complications after myocardial infarction.

Precautions

This medication can cause severe harm to the fetus when taken during pregnancy, especially in the second and third trimesters, so it must be discontinued immediately if pregnancy is confirmed. Pregnant or nursing women, and individuals allergic to this medication or its components should not take it. Patients with severely impaired liver or kidney function and those with a history of certain vascular edema should also avoid this medication. Elderly patients, those with high potassium levels, or mildly impaired liver function should take this medication with caution and may require dose adjustments. This medication can have a strong blood pressure-lowering effect, so caution is needed as dizziness or fainting may occur when starting treatment. When taken with other antihypertensive medications or diuretics, there is a risk of blood pressure falling too low, so it should only be taken under medical supervision. During treatment with this medication, it's important to regularly monitor kidney function and potassium levels through blood tests. If dizziness or symptoms of orthostatic hypotension occur, it is safe to avoid driving or operating machinery. Taking potassium-sparing diuretics or potassium supplements may cause dangerously high potassium levels in the blood, so caution is advised. In case of overdose, severe hypotension or decreased consciousness may occur, and immediate medical attention is necessary. Safety in children or adolescents under 18 has not been established; therefore, use is discouraged. This medication should be kept out of reach of children and should not be transferred to other containers.
