Improvement of essential hypertension
Essential hypertension is a condition where blood pressure is persistently elevated, placing a burden on the cardiovascular system. This medication helps stabilize blood pressure by directly regulating the causes of essential hypertension. By lowering blood pressure to an appropriate level, it effectively reduces the burden on the heart and blood vessels, preventing complications. Consistent use maintains blood pressure control and improves overall cardiovascular health. Therefore, it is used for the treatment of essential hypertension.
The recommended initial dose is 10mg once daily, regardless of meals. If blood pressure is not adequately controlled, it may be increased to 20mg once daily. If additional blood pressure reduction is needed, the dose may be increased to a maximum of 40mg once daily. The blood pressure-lowering effect appears within 2 weeks after starting treatment, with maximum effect around 8 weeks. If blood pressure is not controlled with this medication alone, it may be combined with thiazide diuretics such as hydrochlorothiazide. To improve adherence, it is recommended to take it at the same time each day (e.g., in the morning). In patients who may have reduced effective blood volume (e.g., those receiving diuretics, especially renal impairment patients), treatment should be initiated at a low dose with sufficient monitoring. <For the elderly> No dose adjustment is needed when this medication is administered to elderly patients aged 65 years and older. Overall, there have been no significant differences in efficacy and safety between elderly and younger patients, but a more sensitive response in some elderly patients cannot be excluded. <For patients with renal impairment> In patients with mild to moderate renal impairment (creatinine clearance of 20-60mL/min), the maximum dose is 20mg once daily. <For patients with liver impairment> Patients with mild liver impairment do not require dose adjustment. The recommended initial dose for patients with moderate liver impairment is 10mg once daily, which may be increased to a maximum of 20mg once daily. There is no experience with severe liver impairment, so its use is not recommended. <For children> Safety and efficacy in children have not been established.

This medication can cause serious harm or death to the fetus and newborn if taken during the second and third trimesters of pregnancy, so it should be stopped immediately if pregnancy is confirmed. Persons allergic to any of the ingredients in this medication, those who are pregnant or breastfeeding, patients undergoing kidney dialysis, those with severe liver dysfunction, and those with biliary obstruction should not take this medication. Individuals with lactose intolerance should also avoid this medication, and caution is needed for certain diabetic patients or patients with significantly decreased kidney function. Patients with decreased blood volume or low sodium may experience symptoms of hypotension when first taking this medication, so they should be closely monitored. In patients with severe heart failure whose kidney function relies on the renin-angiotensin-aldosterone system, there is a risk of renal function worsening, so caution is advised. Patients with decreased kidney function or those who have undergone kidney transplantation should regularly check kidney status and electrolyte levels through blood tests. Patients with vascular problems in the kidneys or those at risk of hyperkalemia should carefully monitor serum potassium levels when taking this medication. Patients with aortic or mitral valve stenosis or obstructive heart disease should use this medication with special caution. Due to its blood pressure-lowering effects, this medication may cause dizziness, so it's advisable to avoid hazardous activities such as high places or driving. This medication has generally been used safely in clinical trials; however, mild side effects such as dizziness, headache, back pain, indigestion, and cough may occur. Serious side effects such as angioedema, liver dysfunction, hyperkalemia, and acute kidney failure have been reported rarely, so if any abnormal symptoms occur, consult a physician immediately. If gastrointestinal symptoms like chronic diarrhea or weight loss persist, discontinue use and consult a physician. When taken with potassium-sparing diuretics, potassium supplements, or certain anti-inflammatory pain relievers, serum potassium levels can rise dangerously, so caution is needed with concurrent use. Co-administration with lithium preparations may increase lithium concentration and lead to toxicity, so it is advisable not to take them together. Taking this medication with other antihypertensive drugs may enhance the blood pressure-lowering effect, so consult healthcare professionals. Pregnant women should be informed of the serious risks to the fetus, so if planning for pregnancy or already pregnant, ensure to inform healthcare providers. Breastfeeding mothers should consider discontinuing breastfeeding or this medication as it may pass into breast milk. Safety for children has not been established, so it should be administered to children with caution. In elderly patients, there may be increased drug concentration in the body, so dosage adjustments and careful monitoring are necessary. In case of overdose, symptoms such as hypotension, palpitations, and bradycardia may appear, and immediate medical attention should be sought if symptoms are severe.
