Improvement of essential hypertension
Essential hypertension is a condition characterized by persistently high blood pressure, and this medication is used to manage blood pressure and prevent complications.
The recommended initial dose for adults is 10 mg once daily, regardless of meals. If blood pressure is not adequately controlled, it may be increased to 20 mg once daily, with a maximum dose of 40 mg per day. The blood pressure-lowering effect is expected to appear within 2 weeks of starting treatment, with maximum effectiveness usually seen at around 8 weeks. If the blood pressure-lowering effect is insufficient, this medication may be used in combination with thiazide diuretics. It is recommended to take this medication daily at the same time (e.g., in the morning) to enhance adherence. For patients at risk of intravascular volume depletion (e.g., patients on diuretics, particularly those with renal impairment), treatment should be started at a lower dose with careful monitoring of their condition. <Elderly> No dose adjustment is required when administering this medication to elderly patients aged 65 years or older. Overall, there have been no observed significant differences in efficacy and safety between elderly and younger patients, although it cannot be ruled out that some elderly patients may exhibit more sensitive responses. <Renal Impaired Patients> Patients with mild to moderate renal impairment (creatinine clearance 20-60 mL/min) should not exceed a maximum dose of 20 mg once daily. <Liver Impaired Patients> Patients with mild liver impairment do not require dose adjustment. The recommended initial dose for patients with moderate liver impairment is 10 mg once daily, which can be increased to a maximum of 20 mg daily. There is no experience with severe liver impairment patients, so its use is not recommended. <Children> Safety and efficacy in children are not established.

This medication may cause serious harm to the fetus and newborn if taken during the second or third trimester of pregnancy, so it should be discontinued immediately upon confirmation of pregnancy. Do not use if you are allergic to any of the components in this medication, or if you are pregnant or breastfeeding. Patients undergoing renal dialysis or those with severe liver dysfunction, as well as those with biliary obstruction, should not take this medication. Patients with lactose intolerance should avoid this medication due to the presence of lactose. Patients with certain diabetes conditions or moderate to severe renal function impairment should not take this medication in conjunction with certain other medications. Patients with decreased blood volume or salt may experience low blood pressure when starting this medication, so careful monitoring of their condition is necessary. Patients with severe heart failure or those with renal function that depends on the renin-angiotensin-aldosterone system should be cautious, as changes in renal function may occur. Patients with impaired renal function or kidney transplant recipients should have their renal function and electrolyte levels periodically checked with blood tests, and this medication is not recommended for patients with severe renal impairment. Patients with renal artery stenosis may also experience deterioration of renal function when taking this medication, so caution is advised. This drug can increase serum potassium levels, and patients with heart failure or renal impairment should have their potassium levels regularly monitored. Patients with aortic or mitral stenosis, or obstructive heart disease should be cautious, as blood pressure lowering effects may worsen their symptoms. Patients with primary aldosteronism may not respond well to this medication, so it is not recommended for them. As blood pressure may drop, dizziness may occur, so it is advised to avoid driving or operating hazardous machinery. This medication may be less effective in lowering blood pressure in Black patients compared to other races. During treatment with this medication, mild side effects such as dizziness, headache, dyspepsia may occur, and rarely, angioedema with swelling of the face or throat may occur. Liver function anomalies have been reported rarely, so monitoring liver health is advisable. This medication can interact with other drugs, so caution is needed when used together with potassium-sparing diuretics, potassium supplements, lithium, non-steroidal anti-inflammatory drugs, etc. Diabetic patients or those with decreased renal function are particularly advised to avoid the combination of this medication with certain other drugs. If taken during pregnancy, it may cause serious harm to the fetus, so it should be discontinued immediately upon learning of the pregnancy, and fetal status should be monitored regularly if needed. Breastfeeding women should discontinue breastfeeding or the medication, as this drug may be passed into breast milk. Safety has not been established for use in children, so it is advised not to use this medication in pediatric populations. Elderly patients may have an increased concentration of the drug in the body, so more careful observation is needed during administration. Overdose may cause symptoms of low blood pressure or cardiac arrhythmias, requiring immediate treatment.
