This medication is primarily used to treat acute diarrhea and chronic diarrhea.

How to take

Adult dose

Acute diarrhea: The initial dose of loperamide hydrochloride is 4mg orally, and the maintenance dosage is 2mg administered after each loose stool. The usual daily dose is 6-8mg, and the maximum daily dose is 16mg. Chronic diarrhea: The initial dose of loperamide hydrochloride is 4mg orally, and the maintenance dosage is 2mg administered after each loose stool until diarrhea is resolved. Then, it should be tailored according to the patient’s individual needs.

Child dose

Acute diarrhea: The initial dose of loperamide hydrochloride is 2mg orally, and the maintenance dose is 2mg administered after each loose stool. The maximum daily dose is 6mg.

Precautions

This medication should not be used in infants under 24 months of age or in children with chronic diarrhea. It should also not be administered to patients with bacterial diarrhea accompanied by blood or high fever, patients with acute ulcerative colitis, and those with serious bowel issues such as bowel obstruction. Patients who have had allergic reactions to this medication or who have hemorrhagic colitis should also avoid its use. Additionally, patients with lactose intolerance or related genetic issues should not take this medication due to its lactose content. Patients with liver dysfunction should use this medication with caution, and it should be used carefully if there is an allergy to Yellow No. 5 dye. Possible side effects such as nausea, vomiting, headache, dizziness, and constipation should be closely monitored. If diarrhea does not improve within 48 hours or if it is chronic, it is important to consult a physician or pharmacist if used for more than 10 days. Since diarrhea can lead to loss of fluids and electrolytes, especially children require proper fluid replacement. This medication may affect the central nervous system, so caution should be exercised when driving or operating machinery due to potential fatigue, drowsiness, or dizziness. Pregnant women should use this medication cautiously in early pregnancy and it is not recommended during breastfeeding. Elderly patients may have decreased bodily functions; therefore, dose adjustments and caution are necessary. In cases of overdose, there is a risk of central nervous system suppression symptoms and cardiac arrhythmias, so the prescribed dose must be strictly observed. In cases of overdose, naloxone can be used as an antidote and the central nervous system state should be monitored for at least 48 hours. The administration of tablets or capsules is not recommended for children under 7 years of age. This medication does not treat the underlying cause of diarrhea; therefore, it is important to consult a specialist if symptoms do not improve.

Serious side effects

Hives, angioedema, toxic epidermal necrolysis, erythema multiforme, Stevens-Johnson syndrome, anaphylactic shock and anaphylaxis (severe allergic reactions), bowel obstruction, abdominal distension, toxic megacolon, acute pancreatitis, urinary retention, symptoms of central nervous system suppression (delirium, pinpoint pupils, muscle rigidity, stupor, respiratory depression, coordination dysfunction, drowsiness), QT interval prolongation, severe ventricular arrhythmias, and cases of death.
