It has been found that the number of employed health insurance subscribers who earn more than 20 million won annually through additional income such as interest, dividends, and rental income, in addition to their salary from work, has exceeded 800,000.
On the 26th, Kim Sun-min, a member of the National Assembly's Health and Welfare Committee from the Jo Kuk Innovation Party, reviewed the data titled "Annual Health Insurance Subscribers and Premiums Imposed over the Past Three Years" received from the National Health Insurance Service. According to this data, when calculated based on monthly health insurance premiums, in 2024, there were 804,951 high-income employed individuals earning over 20 million won annually from dividends, rental income, and other sources excluding their salaries. This accounts for about 4% of the total 19,883,677 employed subscribers.
They pay income monthly insurance premiums, also called "salary (compensation) plus insurance premiums." Income monthly insurance premiums are paid separately from the monthly salary-based insurance premiums (which are calculated by dividing the total annual salary by the number of months worked). These premiums are levied on the combined taxable income, including interest from deposits, stock dividends, and rental income.
Initially, the monthly insurance premium was only paid if the comprehensive taxable income outside of salary exceeded 72 million won annually. Then, starting from July 2018, when the first stage health insurance fee assessment system was reorganized, the income threshold for assessment was lowered to 'exceeding 34 million won annually.'
Starting from September 2022, the health insurance premium assessment system was reorganized to a two-tier structure, resulting in a higher threshold of over 20 million won annually, thereby strengthening the assessment criteria.
However, to prevent situations where individuals pay excessively high monthly income-based insurance premiums just because they slightly exceed the standard, additional premiums are only charged on the 'excess amount' for income outside of wages that exceeds 20 million won annually.
These high-income workers are additionally paying approximately 152,000 won per month in health insurance premiums.
As the criteria for calculating insurance premiums based on monthly income continue to be strengthened, the number of employed subscribers paying premiums outside of wages has increased each year, from 194,738 in 2019 to 229,731 in 2020, 264,670 in 2021, 587,592 in 2022, and 662,704 in 2023.
