Because Mr. A's condition was detected early, he did not develop complications such as ulcers or gastric bleeding caused by the bezoar. He has now been discharged. Mr. A stated, "Recently, I often ate tropical fruits that look similar to figs," and "especially in the morning on an empty stomach." Medical staff explained, "Eating foods high in fiber and tannins, such as persimmons, guava, figs, and bamboo shoots, can lead to the formation of gastric bezoars," and added, "Eating them on an empty stomach makes it easier for the bezoar to precipitate and adhere to the stomach wall." Tannins are a type of polyphenol, an antioxidant substance, which gives a bitter taste when consumed.
Calculi can be classified into plant calculi and hair calculi depending on the causative substance. Calculi can also occur after swallowing foreign materials such as skin, plastic, or paper. If a gastric stone remains in the stomach for a long time, it can damage the stomach wall, potentially causing gastric ulcers or bleeding, and may also lead to anemia. Additionally, gastrointestinal obstruction can occur, sometimes requiring surgery.
Treatment of gallstones includes removal via endoscopy, surgery, and medication. The method of removal using endoscopy typically involves cutting the gallstones with tissue forceps or a snare before removal. Methods such as laser-assisted electrohydraulic lithotripsy to crush and then remove the stones are also used.
Methods of removing gallstones using medication include dissolving the stones with proteolytic enzymes such as cellulase and drinking cola. Additionally, there have been methods introduced where cola is injected into the gallstones via endoscopy, followed by removal of the stones. However, if the stones are not crushed during treatment or if the stones pass through the stomach into the small intestine causing intestinal obstruction, surgical removal is necessary.
